Utica Family Support Worker- $50,476 +$2,000 Relocation Bonus

SUMMARY OF RESPONSIBILITIES:Under the direction of the Vice President of the CBI Programs and direct supervision of the Director of Cayuga Family Resiliency, the social worker/therapist will provide intensive home, school and community based clinical, crisis intervention and case management services to families with children in Cayuga County.

FUNCTIONS:

  • Carry a full caseload of 10 families assigned
  • Provide home-based, solution focused case management support and/or crisis interventions to individuals and families both as primary and back-up family worker
  • Actively participates as part of a team of service providers and others with focus on well coordinated services for family members
  • Assists individuals and families by actively referring them to appropriate longer-term services and support
  • Must be ready, willing and able to assist the families with any and all tasks needed to stabilize them with the intention of modeling and teaching basic life skills to empower individuals and families to become self-sufficent
  • Performs the basic accountability tasks related to the job which include compiling and maintaining all necessary records, forms and reports
  • Provides on-call support as assigned
  • Assists administrative staff in collection of information for program effectiveness
  • Provide crisis interventions defusing problem situations on a 24-hour-day on-call basis
  • Coordinates and facilitates parent group/Adolescent group
  • Attend required training.
  • Read required books.
  • Responsible for complying with rules and regulations governing access to Protected Health Information under HIPAA. Access to Protected Health Information is limited to the extent required to perform responsibilities effectively and efficiently. No access will be permitted unnecessarily. The essential job functions as described, herein, dictate the level of access, use, and disclosure of confidential information.
  • Perform other duties as requested by your immediate supervisor or the Chief Executive Officer.

 

Required Experience:

 

  • Masters in Social Work or other Human Service related field with two or more years’ experience in family and children’s services with experience working from a strengths perspective with families.
  • Ability and willingness to work with families in an "in-home" context.
  • Ability to internalize and work within the general "Home Builders" philosophy and program model.
  • Knowledge of family systems theory and home based interventions.
  • Ability to engage families who may not want the service or who are mandated.
  • Ability to have flexibility in hours worked both routinely, so as to meet children and families when they are available, and occasionally, in response to crises which may occur at any hour of any day.
  • Ability to handle confidential material in a responsible manner.
  • Valid Driver’s License required.
  • Vehicle required: Must be safe and dependable.

HOURS PER WEEK: Exempt

  • Regular hours entail 9:00am - 5:00pm, Monday - Friday. Must be flexible to meet program needs.
  • Evening work is needed regularly; Weekend work occasionally.
