Description
As a Family Support Worker, you will use your unique experience raising or having raised a child with a mental health concern to provide parenting education, support and activities aimed at improving the health and well-being of school-aged children and their families.You will report to the Senior Director of Behavioral Health. You will work onsite, splitting time between Roots Academy at West Hill and Sheridan Preparatory Academy, with some travel required to community-based locations.
Responsibilities:
- Foster positive and collaborative relationships with parents and guardians to promote students’ emotional, physical and behavioral development.
- Provide empathetic, patient, and non-judgmental support to children and families.
- Offer guidance, resources and empowerment to families, helping them understand and address the health needs of both themselves and their children.
- Maintain familiarity with community resources and services available to families.
- Collaborate with community organizations to connect families with additional resources and services in the school community.
- Facilitate family support groups, workshops and parent training sessions focused on mental health awareness, positive parenting strategies and building support systems within their communities.
- Participate in school support and multidisciplinary team meetings to discuss student progress, concerns and intervention strategies.
- Organize and lead student lunch groups and summer activities for students enrolled in school-based behavioral health services.
- Maintain accurate and up-to-date records on student progress, family interactions and support activities.
- Contribute to the school-based behavioral health team through participation in staff meetings and training opportunities.
Requirements and Education:
- High School Diploma or equivalent required.
- 1 or more years of experience working with children in an educational or mental health setting required.
- Personal experience as a parent of a child with mental health needs required.
- Knowledge of child development, mental health disorders, and support strategies for emotional and behavioral challenges.
- Valid NYS driver's license and reliable transportation required.
- Flexibility in work hours required.

Description
As a Service Navigator, you will provide individual mentoring, holistic service planning, and coordinated case management, and complete any necessary referrals/linkages to services. You will work together with Albany County Probation. You will coordinate, facilitate, and track the issuing of cash incentives that will be provided to youth as part of their service plan. You will maintain record-keeping for each youth, including progress notes and service plans. You will provide direct mentoring and case management to each youth and family on their caseload. You will report to the Assistant Director of Juvenile Justice for Prevention and will work a hybrid schedule with 50% community travel.
Responsibilities:
- Manage a full caseload of probation-involved youth.
- Attend routine team meetings with youth and families and Albany County Probation.
- Establish positive and professional relationships with youth and families.
- Track youth involvement in the program and issue incentive stipends as directed by the supervisor. Maintain monitored records of funds issued.
- Create coordinated service plans, to include input from youth, caregivers, subcontracted staff, and Probation.
- Have knowledge of community services in Albany County.
- Help families obtain services they need by making referrals, providing transportation, accompanying them to appointments, and assisting with applications (medical, schools, and DSS).
- Maintain good working relationships with Albany County Probation/DCYF.
- Provide individual mentoring to each assigned youth, at least once per week.
- Maintain cultural sensitivity, competence, and client confidentiality.
- Other responsibilities, as assigned.
Requirements and Education:
- High School Diploma and two years relevant experience or Associate's Degree and one year relevant experience required

Family Peer Advocate
As a Family Peer Advocate, you will help children with emotional and mental health challenges through support and advocacy with parents and caregivers. You will report to the Program Manager, Community Solutions and Peer Support and you will work onsite with travel required.
Responsibilities:
- Provide support, advocacy and resources to the family based on the child's needs as identified by the parent/caregiver
- Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
- Work with family members to identify goals and objectives of treatment
- Educate parents/caregivers on how to advocate if services are not meeting their needs
- Participate in relevant treatment meetings and attend all required trainings
- Complete all documentation
Requirements and Education:
- Family Peers have identified lived experience as a parent or primary caregiver of a child with mental health/behavioral health/special education challenges.
- Provisional Family Peer Credential required, with Professional Family Peer Credential to be obtained within 18 months.
- Minimum of HS Diploma or equivalency.
- Valid NYS Driver's License required
