Responsibilities
Responsibilities:
- Authorizes entry of individuals into facility and directs to desired locations.
- Monitors security signal systems in security office or on doors. Takes appropriate action if warning signal is received.
- Locks and unlocks doors according to schedule.
- During rounds, makes notes of and later reports unsafe conditions and fire hazards observed during rounds.
- • Notifies appropriate authorities within the facility when an incident of significant has occurred. In addition to the Chief of Security, it is imperative that the Nursing Supervisor on duty, or if so directed, the administrator on call be provided with full particulars of the incident.
- Responding to floors and units when called to enforce visiting policy when problems are encountered.
- Operates in accord with package pass procedures.
- Operates the paging system after switchboard service ends during evening shift. Locates and transmits important calls or messages to appropriate calls or messages to appropriate person.
- Controls keys for various offices and shops.
- Is responsible and cooperative with supervisors, fellow employees, residents and families.

Responsibilities:
- Responsible for providing comprehensive primary care, coordination of 24-hour care, medical coverage, and directly provides services to ArchCare participants.
- Provides face-to-face comprehensive History and Physical assessments on new referrals. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants.
- Provides evaluation and treatment of participants with acute illnesses.
- Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.
- Documents changes in the participant's condition and details care provided by completing all required documentation.
- Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care.
- Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate.
