- Collaborate with other members of resident care teams to determine if additional or changes to treatment are necessary.
- Review Wound MD notes for content, provide education on CMS Regulations and Documentation of Wounds and Pressure Ulcers according to NPUAP Guidelines and the MDS RAI Manual.
- Oversee overall wound program education delivery at each SNF, ensuring that areas of concern are addressed and education is provided to direct care staff.
- Educate resident and family members on wound care and infection and injury prevention.
- Partner with DON/ADON to audit MARs TARs for omissions.
- Conduct weekly rounds to discuss measurements, treatments, and audit improving/worsening wounds.
- Perform audits of nutrition notes, admission nursing assessments, MD notes for Pus, refusal of care, etc.
