Responsibilities
The Clinical Nurse Manager is responsible for assisting in the implementation of home health clinical services within the Certified Home Health Agency (CHHA) in a defined area in compliance with applicable laws, regulations and Agency policies. The Supervisor supervises and evaluates assigned professional and para-professional staff in the local office.

Responsibilities
- Under the supervision of the Medical Director, PACE, serves as a liaison between the interdisciplinary team and the community-based physician.
- Nurse Practitioner Community Liaison (NP) who will work in collaboration with contracted community-based primary care providers (CBPCP) to ensure proper oversight and coordination of participant (patient) care with the Interdisciplinary Team.
- The NP will work with the community providers to coordinate and support the participant’s functional, clinical, and psychosocial needs, with an emphasis on independence and wellness.

Responsibilities:
- Responsible for conducting pre-enrollment assessments for prospective enrolleesutilizingclinical nursingexpertiseand critical thinking skills.
- Determinesclinical eligibility and whether the prospect meets the criteria forservices.
- Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.
- Will be expected to complete pre-screening/assessment and complete appropriate paperwork.
- Assessments will be conducted in all service areas.
- Travel to all service areas.
- Will make plan to plan calls when indicated.
- Responsible for environmental assessments.
- Will complete semi-annual assessments.
- Participates incross functionalmeetings.
- Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
- Utilizes a skillful approach for assessing,coordinating,and evaluating consumer needs and development ofinitialcare plan.
- Responsible for clinical evaluation of all new participant referralsfor appropriateness of therapy.
Job Type: Full-Time
Salary: $90,000 to $109,000 per year
