Responsibilities

Responsibilities:

• Responsible for providing comprehensive primary care, coordination of 24-hour care, medical coverage, and directly provides services to ArchCare participants.
• Provides face-to-face comprehensive History and Physical assessments on new referrals. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants.

• Provides evaluation and treatment of participants with acute illnesses.
• Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.
• Documents changes in the participant's condition and details care provided by completing all required documentation.
• Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care.
• Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate.

• Keeps confidentiality of participant records, reports and discussions
• Attends and participates in scheduled staff meetings and participant care meetings as requested.
• Maintains applicable licensure and certification and pursues professional growth through continuing education process.

Job Type: Full-Time

Salary: $250,000 - 260,000 + 35K sign on bonus

Qualifications

Qualifications:

• 1 year of post graduate experience in Internal or Family Medicine
• Board Certified in Internal Medicine or Family Practice

Education:

• MD Degree
• Completion of residency in Internal or Family Medicine

Responsibilities

The Nursing Supervisor is responsible for the planning,coordination and delivery of quality care services to the client and accountable for the agency’s adherence and compliance to federal, state, and local laws, accreditation, agency policies and standards.

• Uphold ethical nursing standards and develop efficient care methods.
• Provide leadership to maintain standards.
• Monitor changing client needs and document client activities promptly.
• Supervise treatment plan execution and coordinate overall care with resources.
• Maintain accurate client records.
• Engage with community health resources.
• Evaluate and oversee field employee skills and services.
• Participate in employee development and compliance.
• Assist in staff selection and orientation.
• Interpret policies to non-nursing personnel.
• Evaluate position performance, set goals, and support improvement plans.
• Provide hands-on nursing care in emergencies.
• Offer vacation coverage during RN absences at specific locations.
• Perform field visits to multiple locations during RN absences.
• Conduct Orientation and In-Service training for various roles.
• Administer vaccines to staff as needed.
• Serve as the "on-call" nurse during assigned weeks, including weekends, responding to emergencies and client visits.

Job Type:Part Time

Salary: $50.71 per hour

Qualifications

Qualifications:

• Current NY State Registered Nurse License
• 1 year supervisory experience

Education:

• Associate Degree required
• Bachelor’s Degree preferred
