Responsibilities

• Conducts enrollment assessments to determine clinical eligibility.
• Completes UAS - NY Assessment instrument.
• Maintains frequent communication with the Home Care Department and Care Management to keep ArchCare Senior Life a primary option for care delivery.
• Identifies any barriers to implementation of Plan of Care.
• Communicates with the Care Team in conveying information to facilitate any concerns, or answer questions.
• Completes UAS - New York Assessment instrument as assigned to determine continuing eligibility and / or identify any needed changes to Care Plan.
• Performs other related duties, as required.

Job Type:Per Diem

Salary: Fee-for-service structure.$225 per assessment, $250 per new enrollment

Qualifications

• Registered Nurse, currently licensed and registered to practice in New York State.
• UAS-NY Certified
• Bachelor's Degree in Nursing preferred.
• Experience in Home Care, case management, or long-term home care.
• Demonstrated skills in planning, organizing and directing the work of others required.
• Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.
• Proficient with DOH UAS assessment instrument.

Responsibilities

Responsibilities:

• Responsible for providing comprehensive primary care, coordination of 24-hour care, medical coverage, and directly provides services to ArchCare participants.
• Provides face-to-face comprehensive History and Physical assessments on new referrals. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants.

• Provides evaluation and treatment of participants with acute illnesses.
• Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.
• Documents changes in the participant's condition and details care provided by completing all required documentation.
• Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care.
• Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate.

• Keeps confidentiality of participant records, reports and discussions
• Attends and participates in scheduled staff meetings and participant care meetings as requested.
• Maintains applicable licensure and certification and pursues professional growth through continuing education process.

Job Type: Full-Time

Salary: $250,000 - 260,000 + 35K sign on bonus

Qualifications

Qualifications:

• 1 year of post graduate experience in Internal or Family Medicine
• Board Certified in Internal Medicine or Family Practice

Education:

• MD Degree
• Completion of residency in Internal or Family Medicine

• Responsible for conducting pre-enrollment assessments for prospective enrolleesutilizingclinical nursingexpertiseand critical thinking skills.
• Determinesclinical eligibility and whether the prospect meets the criteria forservices.
• Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.
• Will be expected to complete pre-screening/assessment and complete appropriate paperwork.
• Assessments will be conducted in all service areas.
• Travel to all service areas.
• Will make plan to plan calls when indicated.
• Responsible for environmental assessments.
• Will complete semi-annual assessments.
• Participates incross functionalmeetings.
• Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
• Utilizes a skillful approach for assessing,coordinating,and evaluating consumer needs and development ofinitialcare plan.
• Responsible for clinical evaluation of all new participant referralsfor appropriateness of therapy.

Job Type: Full-Time

Salary: $90,000 to $109,000 per year
