Responsibilities

Responsibilities:

• Collaborate with other members of resident care teams to determine if additional or changes to treatment are necessary.
• Review Wound MD notes for content, provide education on CMS Regulations and Documentation of Wounds and Pressure Ulcers according to NPUAP Guidelines and the MDS RAI Manual.
• Oversee overall wound program education delivery at each SNF, ensuring that areas of concern are addressed and education is provided to direct care staff.
• Educate resident and family members on wound care and infection and injury prevention.
• Partner with DON/ADON to audit MARs TARs for omissions.
• Conduct weekly rounds to discuss measurements, treatments, and audit improving/worsening wounds.
• Perform audits of nutrition notes, admission nursing assessments, MD notes for Pus, refusal of care, etc.

Responsibilities

The Clinical Nurse Manager is responsible for assisting in the implementation of home health clinical services within the Certified Home Health Agency (CHHA) in a defined area in compliance with applicable laws, regulations and Agency policies. The Supervisor supervises and evaluates assigned professional and para-professional staff in the local office.

• Carries out the agency’s mission, philosophy, goals and objectives within guidelines of Agency policy and position function.

Responsibilities:

The Nurse Liaison facilitates the transfer of patients from a hospital or skilled nursing facility to the care of the home health agency in compliance with applicable laws, regulations and Agency policies.

• Works with the Hospital team to assess patient's home-care needs and establishs a discharge plan.

• Interviews patients and families to assess the patient's understanding of illness and determine patient's home environment and support system.

• Evaluates patient's level of comprehension of procedures and/or treatment to be performed.

• Explains home health agency services and policies to patient and family.

• Assesses third-party coverage and determines if coverage is sufficient to cover the patient's needs at home.

Responsibilities

• Under the supervision of the Medical Director, PACE, serves as a liaison between the interdisciplinary team and the community-based physician.
• Nurse Practitioner Community Liaison (NP) who will work in collaboration with contracted community-based primary care providers (CBPCP) to ensure proper oversight and coordination of participant (patient) care with the Interdisciplinary Team.
• The NP will work with the community providers to coordinate and support the participant’s functional, clinical, and psychosocial needs, with an emphasis on independence and wellness.
• This role will work as the liaison between the participant/caregiver, the community provider, and plan staff to develop and implement the plan of care, including person-center goal development, communicating changes in participant condition, and facilitating primary care preventativeservices.
• The NP will ensure the participant’s medical record is complete by obtaining or coordinating with other staff to obtain medical records, reports, lab results, radiology results, referral results, etc.
• The NP will also be responsible for providing primary care when applicable including participant examinations/assessments, ordering labs/medications/referrals, communicating with participant/caregivers, etc.

Salary: $148,400 to $158,817

• Responsible for providing comprehensive primary care, coordination of 24-hour care, medical coverage, and directly provides services to ArchCare participants.
• Provides face-to-face comprehensive History and Physical assessments on new referrals. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants.

• Provides evaluation and treatment of participants with acute illnesses.
• Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan into the plan of care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.
• Documents changes in the participant's condition and details care provided by completing all required documentation.
• Collaborates with nurse, nurse practitioner and other physicians to ensure comprehensive and continuous care.
• Assists in monitoring the medical care of enrolled members who are hospitalized in out-of-network and out-of-service area facilities and assists in their transfer to in-network facilities when medically appropriate.

Responsibilities

The Care Manager is responsible for the provision of professional medical care coordination services to patients and their families and provides medical care coordination consultation to patients in compliance with applicable laws, regulations, and Agency policies.

• Establishes and maintains effective working relationships with patients, families, agency staff and other community agencies.
• Gathers pertinent medical and non-medical information from patient/family/staff or from patient record on referred patients.
• Assesses the environment of the home and community to identify elements which impact negatively on the patient’s/family’s ability to function.
• Develops a plan of care in collaboration with the patient, family and other health professionals.
• Provide direct medical care coordination services to patient/family or makes appropriate community/health referrals.
• Conducts comprehensive assessment with clients and/or their affected family members in a timely manners.
• Attends training seminars, staff meetings and supervision on a regular basis and in a prompt manner.
• Helps patients and families to develop capabilities that permit them to make optimum use of health and social facilities.
• Participates in conferencing and planning with other disciplines caring for the patient.
