Responsibilities
- Under the supervision of the Medical Director, PACE, serves as a liaison between the interdisciplinary team and the community-based physician.
- Nurse Practitioner Community Liaison (NP) who will work in collaboration with contracted community-based primary care providers (CBPCP) to ensure proper oversight and coordination of participant (patient) care with the Interdisciplinary Team.
- The NP will work with the community providers to coordinate and support the participant’s functional, clinical, and psychosocial needs, with an emphasis on independence and wellness.
- This role will work as the liaison between the participant/caregiver, the community provider, and plan staff to develop and implement the plan of care, including person-center goal development, communicating changes in participant condition, and facilitating primary care preventativeservices.
- The NP will ensure the participant’s medical record is complete by obtaining or coordinating with other staff to obtain medical records, reports, lab results, radiology results, referral results, etc.
- The NP will also be responsible for providing primary care when applicable including participant examinations/assessments, ordering labs/medications/referrals, communicating with participant/caregivers, etc.
