Responsibilities
- Conducts enrollment assessments to determine clinical eligibility.
- Completes UAS - NY Assessment instrument.
- Maintains frequent communication with the Home Care Department and Care Management to keep ArchCare Senior Life a primary option for care delivery.
- Identifies any barriers to implementation of Plan of Care.
- Communicates with the Care Team in conveying information to facilitate any concerns, or answer questions.
- Completes UAS - New York Assessment instrument as assigned to determine continuing eligibility and / or identify any needed changes to Care Plan.
- Performs other related duties, as required.
Job Type:Per Diem
Salary: Fee-for-service structure.$225 per assessment, $250 per new enrollment
Qualifications
- Registered Nurse, currently licensed and registered to practice in New York State.
- UAS-NY Certified
- Bachelor's Degree in Nursing preferred.
- Experience in Home Care, case management, or long-term home care.
- Demonstrated skills in planning, organizing and directing the work of others required.
- Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.
- Proficient with DOH UAS assessment instrument.
