Adult Community Case Manager:
Description
Independence Association seeks a full-time case manager to join our growing adult case management team. As a case manager, you will play a key role in assisting individuals with intellectual disabilities or autism, by accessing resources to assist them to live a full and inclusive life. You will do this by providing thorough assessments of strengths and needs, comprehensive planning of service needs, advocacy, outreach and resource coordination unique to every person served. You will work closely with both the individual and his or her identified support group, and be a leader in assisting individuals to develop a yearly Person Centered Plan to achieve their goals. Quick synopsis:

Requirements:
• Bachelor's degree (required)
• Experience of 1 year working as an Adult Case Manager (required)
• Personable, self-directed, great written and oral communication skills, team player
• Comfortable advocating for individuals with various levels of disability in a variety of settings
• Travel required (must have valid driver’s license)

Children's Case Manager:
Independence Association is seeking a full-time, energetic case manager to join our dynamic children’s case management team. Our case managers play a key role in assisting individuals with intellectual and developmental disabilities to access resources and live full and inclusive lives. They work closely with both the individual and their identified support group. Our case managers provide thorough assessments of strengths and needs, comprehensive planning of services, advocacy, outreach, and resource coordination unique to every person served. Clinical supervision is provided for all pursuing a social worker license, along with a reimbursement program for professional testing and licensing fees.
Job Duties:
- Manage a caseload assigned by team leader and/or program manager.
- Enter all notes of service into the agency electronic medical records system within established agency timelines.
- Coordinate, schedule, facilitate and/or lead the members of their team to create the ISP.
- Assist individuals and their families to make choices, achieve personal goals, and promote the individual’s inclusion into the community as identified in the ISP.
- Coordinate and facilitate transition and discharge planning.
- Assist and monitor individuals’ efforts to secure and maintain needed services and entitlements such as housing, financial benefits, health needs and educational supports.
Requirements:
• Bachelor's degree (required)
• Experience of 1 year in human services field (required)
• Valid driver’s license (required)
• Ability to work independently
• Great written and oral communication skills
• Comfort advocating for individuals with various levels of disability in a variety of settings
Physical Requirements*:
- Ability to drive a vehicle for up to a half an hour commute to the home of family.
- Ability to type on the computer for up to 4 hours per day with breaks.
