ArchCare Senior Life (PACE) is a long-term Medicaid/Medicare plan designed specifically for seniors who need nursing home level care but want to stay at home. With PACE, participants have a dedicated Care Team to coordinate everything, so they are able to live comfortably where they are, knowing they'll get the coordinated care they need. PACE provides and coordinates care at our Wellness Centers, in patients' homes, and in other settings.

Responsibilities
Responsibilities:
- Responsible forprovidingcomprehensive primary care, coordination of24-hourcare, medical coverage, and directlyprovidesservices toArchCareparticipants where they live, in congregate housing, Adult homes, and home visits.
- Providesface-to-face comprehensive History and Physical assessments on new referrals, and on existing patients. Prepares face-to-face Quarterly or as needed History and Physical assessments on established participants with a full comprehensive face to face exam including pts institutionalized.
- Provides evaluation and treatment of participantswithacuteillnesses, and works with other staff to ensure patient is safe and healthy.
- Attends IDT’s and morning report within your area.
- Participates in the development and revision of the participant's plan of care as a member of the interdisciplinary team. Integrates the primary care treatment plan intothe planof care developed by the interdisciplinary team. Interacts with other team members to meet emergent and acute needs of participants. Participates in discharge planning.
- Documentschangesin the participant's condition and details care provided by completing all requireddocumentation.
- Collaborates withnurse, nursepractitionerand other physicians to ensure comprehensive and continuous care.
- Assistsinmonitoringthe medical care of enrolled members who are hospitalized in out-of-network and out-of-service areafacilities andassistsin their transfer to in-network facilities when medicallyappropriate.
- Attends andparticipatesin scheduled staff meetings and participant care meetings asrequested.
- Participates in meetings for PCP’s such as QI, IC and UM, and updates on CMS rulings.
- Speaking with consultants/Specialists regarding patient care
- Responsible to keeping our EMR inputting progress notes and reviewing consultation recommendations, up to date on a daily basis.
- Participate on call, on call is Friday to Friday x 7 days every 8 weeks

Qualifications
Qualifications:
- 1 year of post graduate experience in Internal or Family Medicine
- Board Certified in Internal Medicine or Family Practice
Education:
- MD or DO Degree
- Completion of residency in Internal or Family Medicine
- Geriatric experience preferred
- Bilingual preferred
Salary
Salary: $240,719.93 per year
