Responsibilities

• Maintain and Update HCHB system with Demographic changes and updates related to physicians.
• Call for Follow Up on Physician Orders/F2F Documentation.
• Maintain and Update HCHB with Signed Physician Orders.
• Ensures appropriate documentation received such as: HHCCN/NOMNC.
• Verify and Maintains receipt of signed/dated documents (Consents).
• Provides Director of Operations and Regional Administrators with assistance with data entry, analysis, and reports.
• Performs general administrative duties such as: nursing supply inventory and distribution, ordering supplies, answering incoming calls, distributing messages, faxing, filing, etc.

• Observes nursing care of the residents to ensure that orders are carried out as directed and treatment is administered in accordance with physician's instructions.
• Assists with orientating newly employed nurses and clinical placement of nursing students.
• Arranges hospital transfer of residents.

• Administers speech and language therapy treatments to patients as per physician's orders using modalities and techniques appropriate for the individual patient.
• Makes an initial and periodic evaluation of the patient's speech and language and rehabilitation needs. Sets long and short-term goals in measurable terms and adjusts treatment plans accordingly.
• Consults with the Rehabilitation Therapies Coordinator and nursing staff to make joint decisions regarding continuation of, or dismissal from care when the need for speech therapist treatment is no longer necessary.

• Assist with the orientation and/or guidance of new staff members professional and non-professional, nursing students, volunteers, and special visitors.

• Responsible for evaluating the patient's level of physical functioning and providing training and/or assistive devices to eliminate or reduce areas of deficit.
• Develops a plan of care with patient and caregiver involvement and in collaboration with the nurse, identifying short and long-term goals.
• Evaluates, instructs and monitors specific functional exercise programs to increase strength, range of motion, and coordination of upper extremities.
• Teaches use of adaptive equipment to the patient, caregiver, and other appropriate health team personnel to assist the patient to achieve maximum independence.

• Maintains communication with the physician and other personnel involved in the patient’s care, through written reports, attendance at meetings, and confirmed telephone calls.

Responsibilities

Assumes professional responsibility for the Home Health Agency and integrated programs. Provides leadership and direction for program services consistent with the overall mission, philosophy and policies of the organization. This involves planning, organizing, directing, monitoring and evaluating, as well as assuring compliance with State, Federal, and Accreditation and Agency standards. Works closely with the Executive Team, appropriate management staff and committees to facilitate program coordination and implementation.
