Description
As a Director of Community Transition Time Program (CTTP), you will oversee a new model of care for youth to keep them out of unnecessary hospital settings. You will manage a multi-disciplinary team that provides services and support to adolescent youth and their families in the community and short-term residential settings. Responsibilities will include the management of the program, staff matters, client referrals, client treatment, financial viability, marketing of the program, interfacing with referral sources including local hospitals and county mental health providers, program manual, and program systems. You will report to the Senior Director of Residential Services and will work onsite.
Responsibilities:
- Lead on recruiting, hiring and onboarding team members.
- Provide overall supervision to the team.
- Monitor caseload size and coordinates program intakes and discharges.
- Provide training and supervision for the Staff.
- Oversee a short-term eight bed residential unit that provides 24/7 care.
- Make rotating on-call schedule for after hours and weekend coverage by team members.
- Become subject matter expert in this new program model and participate in collaborative meetings with the state and other agencies.
- Educate the community about Community Transition Time and Intervention program, outcomes, theoretical background.
- Develop and maintain collaborative relationships with other agencies, schools, colleges, institutions, hospitals, primary care providers, housing authorities, and law enforcement agencies to help meet the needs of this population through timely referrals, connections and warm handoffs.
- Ensure staff are adhering to the fidelity of the program.
- Participate in all required trainings as directed.
Requirements and Education:
- A Master’s degree and license in a helping profession (Social Work, Mental Health Counseling, Marriage and Family Therapy, Creative Arts Therapy, etc.)
- At least two years of progressively responsible related experience.

Our new colleague will teach two courses each year (winter and spring trimesters; early January through mid-June 2026), which are components of our year-longclinical sequence: Clinical Psychology 2: Interventions (with a lab), and Clinical Psychology 3: Internship. Clinical Psychology 2:Interventions covers theories of psychotherapy and communication, and with an experiential labsection. In Clinical Psychology 3: Internship, advanced students spend ten hours perweek at supervised internship sites in the community, and meet one class sectionper week for group supervision.
Many of our students are interested in pursuing careers in the helping professions, andcontinue on to masters, doctoral, or pre-med programs. There would also beopportunities to mentor clinically-bound students, to supervise senior thesis projects,and conduct one’s own independent research, if desired. Visiting faculty and their students areeligible for generous research grant funding offered by the college and conference travel funding. Completion of an APA-approved internship is required, and applicants should be eitherlicensed or license-eligible in the state of New York.

Compensation:
The annual salary for this position is $37,600. The wage/salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The stated hiring rate/range represents the College’s good faith and reasonable estimate of the rate/range of possible compensation at the time of posting.

Description
As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work w/children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review Plan of Care; identifying progress, continued needs, and confirming ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended trainings
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- You have a valid driver's license and automobile that is insured following New York State Requirements
- You will work with individuals of different backgrounds, age, ethnicities, life positions, and socio-economic statuses.
