Liaison to Long Term Care Facility

The Sisters of St. Joseph of Carondelet, Albany Province is a community of 280 women religious who promise to live entirely for God and for others. The center of the Albany Province is St. Joseph’s Provincial House in Latham, New York, which is not only the administrative center for the Province but also home to the Sisters who live and work together. The Provincial House employs approximately 75 lay staff.

We are seeking a Liaison to a Long Term Care facility where some of our Sisters reside. In this position the Liaison will be responsible for the healthcare and financial needs of the Sisters.

Essential Functions of the Position

  • Oversees the care of the sisters in conjunction with other team members.
  • Acts as a liaison with the long-term care facility staff (Nursing, Social Services, PT, Dietary, Scheduling, Business and Maintenance) to ensure the needs of the sisters are addressed.
  • Attend Care Planning Meetings.
  • Communicate changes in healthcare status to appropriate staff and healthcare proxies.
  • Escort sisters to medical appointments.
  • Acts as the on-call contact in the event of hospital emergencies.
  • Prepares budget information for sister residents in long-term care.
  • Coordinates meetings, communications and social/Holiday events.
  • Communicates with family members in coordination with other team members.
  • Sets up and monitors personal phone accounts, television and other needs for sisters.
  • Assists with moving sisters into Long-term care.

Qualifications

  • BA or MA in a field related to health and/or pastoral care.
  • NY Driver's license.

  • Significant understanding of, and respect for, religious life and the Mission and Ministry of the Sisters of St. Joseph
  • Strong ethical behavior and business practices
  • Ability to work across multiple constituencies, including third parties, at multiple levels of the organization, including the executive level
  • Ability to communicate effectively at all levels. Understand and respond to the needs of the Sisters and staff in an appropriate manner
  • Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance the care provided to the sisters.
  • Assess problem situations to identify causes, gather and process relevant information, generate workable solutions, and make recommendations and/or resolve the problem
