Case Manager, Mobile Crisis (23271)
As a Case Manager for Mobile Crisis, you will provide case management services to support the crisis intervention and follow-up plan for each client. These services will be implemented using a strength-based, trauma informed, systematic perspective, and will include coordination of services with clients, families, existing treatment providers and linkage to additional community-based therapeutic resources and supports. You will report to the Director of Crisis Services and will work onsite with community-based travel required.
Responsibilities:
Assist the Crisis Services leadership with coordinating all crisis services and perform other duties as directed.
Document all client-related communication, interactions, and contacts in progress notes.
Monitor/Manage Medical Record documentation needing to be attached to a client’s medical record
Assist the Crisis Services leadership in maintaining compliance of Medical Records.
Consult/Collaborate with community agencies to identify potential resources for our clients and families
Coordinate community referrals needed/recommended by the Mobile Crisis Team Leads
Monitor/Manage Data collection for reports and presentations as directed by Crisis Services leadership
Run statistical findings from data collection for Reporting purposes.
Monitor/Manage sufficient supplies, vehicles and building related operational needs for the program operations, submitting needs requests to the Director of Crisis Services.
Assist and monitor program billing practices
Other duties as assigned.
Requirements and Education:
Requires a Bachelor’s degree and at least one year of related experience.
A valid driver's license
