Description
As a Part-Time Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. You will report to the Case Manager Supervisor. You will work in a hybrid environment with some travel to client's homes in the community.
Responsibilities:
- Function as a systems navigator to families who have adopted or who have children in guardianship or kinship care through the provision ofcase management.
- Meet with families in their home to assess needs
- Assess needs, conduct service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment and review with the family via in-home or community monthly visits. Services include but are not limited to information and referral services; referral to legal services; attendance at family court dates; transportation; housing assistance; childcare; basic family needs (vital needs in emergencies and with expenses that are unforeseen); educational advocacy services; and facilitating appointments with social services, medical health providers, and mental health services.
- Respond to the request for in home support referrals for resources and services
- Provide crisis intervention supports when needed
- Work with family to access needed respite services
- Conduct follow up contacts with families at prescribed intervals
- Participate in family engagement activities
- Co-lead support groups
- Participate in training programs offered by the program as assigned
- Use lived experience to empathize and provide hope. Preference for someone with experience with kinship, adoption or guardianship as a youth or in a parenting role.
- Maintain records including required data collection information according to the program standards and expectations. Case Managers must maintain Evolv record with progress notes and all pertinent documentation, a discharge summary and post-case closure documentation.
- Participate as a trainer in programs in the community, to increase understanding of the needs of families who have adopted or have children in guardianship or kinship care
- Complete risk assessments as required by Northern Rivers Quality Management Department
Requirements and Education:
- Bachelor’s level degree in social work, psychology, or human services, or a bachelor’s level degree in any field with at least two years of case management experience; or an associate’s level degree in psychology or human services with at least two years of case management experience.
- Must be able to communicate effectively and develop positive working relationships
- A valid NYS drivers license

Director Nursing - Sign-On Bonus & Relocation Assistance Available
As a Nursing Director, you will lead medical services delivery within the division of Residential and Foster Care Services for Northern Rivers. This also includes oversight of nursing services provided in the agency schools and day treatment. You will oversee policy and protocol development and implementation for health service delivery, and will work with the Medical Director to ensure medical services are provided according to all applicable standards and regulations. You will report to the Chief Officer, Residential & Foster Care Services and you will work on the Albany campus with expectation of some visits to the Schenectady campus.
Responsibilities:
- Maintain appropriate staffing levels for medical services including hiring, staff development and retention of staff.
- Oversee policy and protocol development and implementation for health service delivery for the division
- Review existing practices to find efficiencies and establish common protocols between programs when possible.
- Develop relationships with health providers, schools of nursing, and possible services that the agency will contract with for healthcare
- Maintain a strong working relationship with agency leadership including chiefs, directors, and central services.
- Oversee management of billing and managed care relationships relative to these health services.
- Oversee compliance of medical records with all applicable standards and implement recommendations of other quality improvement and regulatory bodies (i.e., COA, IOC, OCFS, SED, OMH, etc.)
- Participate as a member of multi-disciplinary teams with residential services, foster care and the education department in support of coordination of care of youth.
- Position takes part in rotating telephonic on call to support Nursing.
Requirements and Education:
- Bachelor's degree (BA/BS) from a four-year college or university
- 5 to 7 years of management or supervisory experience
- RN License in New York required.
- Must pass physical.

The Youth Assertive Community Treatment (ACT) program is seeking a Psychiatric Nurse Practitioner. This position is responsible for providing comprehensive Psychiatric services to adults and youth who are enrolled in our outpatient clinic program and a 28-person caseload of youth enrolled in our Youth ACT program. This position is full-time, 40 hours per week. Must be able to seechildren.
Responsibilities:
Provide comprehensive psychiatric services to youth enrolled in Youth ACT and adults and youth enrolled in our BehavioralHealth Clinic as assigned
Provide consultation to staff on diagnostic and treatment questions
Participate in formal and informal training and education to staff
Prescribe medications, monitor, and review of drug regimen for enrolled individuals
Participate in quality assurance activities, consult with the Youth ACT Team in developing and reviewing service plans for all Youth ACT enrolled individuals
Provide clinical expertise, consultation, and technical assistance to the Youth ACT Team/Behavioral Health, cooperate with the County and participate in any quality audit and utilization review
Provide on-site/community based/home visits medication evaluations and reviews for Youth ACT enrolled participants.
Education and/or Experience:
Requires Board Certification by the American Board of Psychiatry and Neurology. Must have a minimum of one year experience working with youth and adults diagnosed with mental illness.
Must be fully licensed Nurse Practitionerin the State of New York.

Job Summary | The Lally School of Management at Rensselaer Polytechnic Institute seeks applications for a tenure-track faculty position starting Fall 2025, at the Assistant or Associate Professor level, from scholars who work in areas at the cutting edge of finance, quantitative finance, financial economics, and/or financial analytics, as they pertain to the digital transformation of the financial services industry. Candidates must have a PhD degree in a relevant business discipline from an accredited university along with demonstrated evidence of producing original and innovative scholarly work with publications in premier business journals. The successful candidate will join Lally either as a tenure-track assistant professor, or as an associate professor, based on teaching and research experience and success. The successful candidate will have a demonstrated potential and commitment to effective teaching in our graduate and undergraduate courses in basic and advanced topics in analytics for finance and accounting, as well as provide service to the Lally School of Management and to Rensselaer. As a Mobile Clinical Site Supervisor, you will oversee daily management and oversight of all staff to ensure management of the crisis line and onsite response to all 6 counties is coordinated. You will report to the Director of Crisis Services and you will work on-site.
Responsibilities:
- Provide daily management and oversight to staff including:
- Manage the crisis line.
- Monitor triages to ensure appropriate call management response.
- Assist with coordinating team response based on staffing per call.

POSITION OVERVIEW
As a Family Support Worker, you will work as part of the Short-term Services Program which was developed as an intensive case-management service to rapidly resolve the concrete and referral needs of families entering the Child Welfare System. By successfully resolving these issues within 90 days of admission, families are at a reduced risk for being referred to Long-term CPS.
