As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work w/children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review Plan of Care; identifying progress, continued needs, and confirming ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended trainings
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- You have a valid driver's license and automobile that is insured following New York State Requirements
- You will work with individuals of different backgrounds, age, ethnicities, life positions, and socio-economic statuses.

Are you a parent or custodial caregiver who is currently or has raised a child with serious mental health concerns?
Are you a parent or custodial caregiver that is personally familiar with the associated challenges related to your child’s mental health diagnosis?
Would you be interested in a job where you can personally guide other parents and caregivers through the support and steps available to take to promote stability and whole-family health?
If you answered “yes” to these questions – Northern Rivers Family of Services would love for you to consider joining our team as a Family Peer Advocate!
As a Family Peer Advocate, you will support youth and parents/caregivers through the referral, orientation, and navigation process related to the Residential Treatment Facility. You will report to the Director of RTF and you will work onsite with travel required.
Responsibilities:
- Upon referral, schedule face-to-face contact with the parent/caregiver of youth being referred. The purpose is early engagement and to explain what to expect with the pre-admission and admission process.
- Work with the youth, family, and RTF Transition Coordinators on early engagement, and treatment planning, and discharge planning with post follow-up.
- Participate in all pre-admission meetings, admission day events, treatment team meetings, red flag, and other meetings with the parent/caregiver of youth on case load.
- Conduct the Family Assessment of Needs and Strengths (FANS) within 30 days of the youth being admitted and every 90 days thereafter.
- Complete the FANS form, Family Plan, and individual tracking sheet for each family.
- Develop goals of the FANS, and document progress on the goals on an ongoing basis.
- Conduct monthly and more frequent meetings with the parent/caregiver to review and work on goals.
- Provide transportation to parent/caregiver for meetings.
- Perform home visits.
- Provide linkages to the parent/caregiver's county-level family support services program.
- Provide emotional support to parent/caregiver.
- Help parent/caregiver navigate the residential system.
- Provide consultation to staff regarding working with families of children with emotional difficulties.
- Maintain appropriate records for each family.
- Stay current with PEP.
- Participate in trainings that support the role: BBI, Coalition, and Wrap around trainings.
Requirements and Education:
- Requires an Associate's degree and one to two years of related experience, or equivalent education and experience.
- Must be a parent of a child with an emotional disability; experience advocating for services in a residential setting desired.
- Driver's license is required.

As a Family Peer Advocate, you will help children with emotional and mental health challenges through support and advocacy with parents and caregivers. You will report to the Program Manager, Community Solutions and Peer Support and you will work onsite with travel required.
Responsibilities:
- Provide support, advocacy and resources to the family based on the child's needs as identified by the parent/caregiver
- Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
- Work with family members to identify goals and objectives of treatment
- Educate parents/caregivers on how to advocate if services are not meeting their needs
- Participate in relevant treatment meetings and attend all required trainings
- Complete all documentation
Requirements and Education:
- Family Peers have identified lived experience as a parent or primary caregiver of a child with mental health/behavioral health/special education challenges.
- Provisional Family Peer Credential required, with Professional Family Peer Credential to be obtained within 18 months.

PEER ADVOCATE - ASSERTIVE COMMUNITY TREATMENT
Rensselaer, NY
As a Peer Advocate, you will be an ally, mentor, and advocate for clients and families as they contend with the difficulties and uncertainties of mental illness. You will use lived experience to contribute to treatment planning for each participant, and facilitating the team’s development and maintenance of a culture of respect. The Youth Assertive Community Treatment team (ACT) Program is an innovative treatment program. The program serves children with mental illness or serious emotional disturbance and their families. You will report to the Director of Youth Act and work onsite with community-based travel required.
This new employee will be hired under the Youth Peer Advocate title or Family Peer Advocate title. The requirements are as follows:
Youth Peer Advocate:
- As this is a Youth Peer position, grant funding requires that employees hired in this capacity are within 18-30 years of age and have lived experiencewith mental illness or serious emotional disturbance.
- The FPA must possess a credential recognized by the Office of Mental Health and receive specialized training and supervision.
Family Peer Advocate:
- Family Peer Advocates (FPA) are parents or caregivers who are raising or have raised a child with serious mental health concerns and are personally familiar with the associated challenges and available community resources for children and families.
- The YPA must possess (or obtain within the first twelve months of employment,) a credential recognized by the Office of Mental Health and receive specialized training and supervision.
Peer Advocate Responsibilities:
• Engage individuals and establish trusting, collaborative relationships directed toward the goals clients identify on their treatment plans
• Contribute to client treatment plans, advocating for clients' needs
• Participate in daily treatment team meetings and work directly with other team members to provide support and assistance to program participants.
• Provide individual face-to-face support to youth and families receiving services
• Spend 70% or more of scheduled work hours in community settings
• Meet with family, youth, and collateral contacts to provide psychoeducation about serious mental illness
• Be a role model, educate recipients about self-help techniques and self-help group processes, teach effective coping strategies based on personal experience, teach symptom management skills, assist in clarifying rehabilitation and recovery goals, and assist in the development of community support systems and networks.
Requirements and Education:
- High School Diploma or GED, Associates Degree preferred
- A valid New York State driver’s license and must have access to a reliable, insured vehicle and be willing to transport clients in the community if needed.
- Complete and obtain the NYCPS-Provisional certification for review by the New York Peer Specialist Certification Board (NYPSCB) within the agreed-upon time of start date.
