Description
As a Clinician, you will coordinate client assessments and work with the child, family, legal guardian, law guardian, psychiatrist, nurse, psychologist, educators, child care staff, and other providers to prepare, implement and monitor a comprehensive, permanency-focused case plan for children in the Residential Program. You will report to the Director of the Residential Treatment Facility and will work on-site.
Responsibilities:
Provide assessment, treatment planning and general counseling
Required to provide 20-25 hours of individual, group, or family therapy per week.
Conduct specialized risk assessments.
Consult with other staff involved in the care of the child and families.
Attend case related meetings.
Participate in supervision.
Have regular communication with birth families around progress/problems of children in care, how to support them and work towards permanency.
Provide regular case documentation for all children and families on workload including assessments, progress notes, treatment plans.
Requirements and Education:
Requires a Master’s degree; related experience preferred.
RTF Clinician requires two years of related experience.
Professional licensure:(LMSW with 3 years of experience, LMHC-D, LMFT,-D LCAT, LCSW) required
Must pass physical.
Must be able to complete TCI training.

Description

- Work with the direct treatment providers to establish treatment goals for children, utilizing historical and current available information.
- Meet and communicate with biological families, guardians, foster families, and case workers.
- Assess individual and family situations.
- Identify problem areas, treatment goals and methods to attain goals.
- Provide therapeutic services to children on at least a weekly basis and families on at least a bi-weekly basis using differential treatment modalities.
- Work with interdisciplinary team to develop and implement a comprehensive treatment plan and to monitor ongoing progress of child and family.
- Learn and practice case coordination skills.
- Work with referring and collateral agencies and prepare reports and correspondence as needed.
- Record and maintain case record in accord with Agency, OMH, and other state mandates.
- Participate in intake assessments as assigned.
- Diligently plan for discharge/permanency, at the time of admission.
- Actively communicate with clients and customers through phone and email correspondence.
- Ensure quality oversight and improvement services.
Requirements and Education:
- Requires a Master’s degree; related experience preferred.

- Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
- Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
- Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
- Attend Behavioral Health Center staff meeting twice a month.
- Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
- Provide referrals and linkages to community supports, resources, and services.

As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.
Responsibilities:
- Complete the nursing observations report findings to the registered nurse
- Review lab and test results, reporting critical values to RN and providers
- Medication management including ordering medications and obtaining prior authorizations
- Dispense medications
- Communicate with medical director/providers regarding observation in client’s health/behavior
- Provide client and family education regarding medications and treatments
- Collaborate with our staff regarding concerns and behaviors
- Participate in Interdisciplinary Medication Review meetings
- Nursing documentation using EHR
- Update discharge planning needs
- Other duties as assigned within scope of practice and based on agency needs
Requirements and Education:
- LPN Licensed in New York State
- 1 to 2 years of experience
- CPR Certification
- Experience working with youth and adolescent population desired
- Must pass physical.
