Are you looking to make a difference in the lives of youth and families struggling with behavioral health issues? Do you want to develop and supervise your own team and mentor your staff to improve their clinical skills? As the Team Lead of Home Based Crisis Intervention (HBCI), you will provide clinical supervision, support, and mentorship to your team to help them change lives. The HBCI team provides short-term (up to 6 weeks), intensive intervention to youth and families who are in crisis. We work with parents and caregivers to help them help their children. The short-term format means the work is fast-paced and outcome oriented, which means you and your team will produce meaningful outcomes in a short period of time. You will report to the Chief of Crisis Services and work on-site with travel required.
Responsibilities:
- Provide clinical supervision to the team and be responsible for allocation of staff time; provide training and ensure all team members are kept informed of issues.
- Monitor caseload size and coordinate program intakes and discharges.
- Create rotating call schedule for after-hours / weekend coverage by team members.
- Provide education and outreach to the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.
- Develop collaborative relationships with other agencies, schools, hospitals, primary care providers, and local government agencies to help meet the needs of this population so they can achieve and maintain their independence.
- Assist primary clinicians to conduct needs assessments, safety planning and wellness management planning with participants. Connect participants and families with the services and support they need. Provide psychoeducation regarding mental health diagnosis and treatment. Develop treatment using a shared decision-making process with clients.
- Assist primary clinicians to respond to on-call needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling cognizant of family needs, with 24/7 availability nights, weekends and holidays.
- Promote safety of the child, family, and community in the delivery of crisis intervention services.
- Assist primary clinicians to conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment while developing a positive working relationship with the families.
- Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves and prepare them for transition and service closure.
Requirements and Education:
- Requires a Master’s degree and at least one year of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.
- Must be a licensed mental health professional. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist.
- Must have a valid driver’s license.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work with children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care

- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.

As a Clinician for Mobile Crisis, you will be providing crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. You will work on-site with travel to the community and individuals' homes.
Responsibilities:
- Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
- Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
- Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
- Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
- Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).
Shift/Schedule:
- Fulltime; 5 days a week 8 hour shifts or 4 days a week 10 hour shifts
- Cross shifts: 10am-6pm, 11am-7pm, or 12pm-8pm
- Part time hours available
- Weekend and holiday expectations
- When scheduled for a weekend shift your schedule will be 5, 8 hour shifts for that week
Requirements and Education:
- Looking for applicants with their LMSW, LMHC, LCSW, LCSW-R, MSW-LP or MHC-LP licensure, must be license eligible and willing to apply for a limited permit in discipline upon hire

Child Health Home Care Manager-Based in Greene and Columbia Counties

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work with children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review the Plan of Care; identify progress, and continued needs, and confirm ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended training
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.

Description
As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.

- Work with family members to identify goals and objectives of treatment

Requirements and Education:
- Bachelor's degree in a related field and at least two years of related experience OR

Description
As a Teacher in Training, you will be responsible for development of classroom instructional plans for students and support student's success in academic and behavioral goals. You will report to the Principal and work on-site.
Responsibilities:
- Develop and implement a classroom instructional plan based on a students IEP and grade and subject area curricula; work with content area teachers to ensure that their lessons are differentiated as to meet the needs of all students.
- Develop and implement Individualized Educational Plans (IEPs) for all students on their caseload. Ensure that accommodations and modifications are provided. Provide specially designed instruction and monitor student progress toward academic and behavioral goals. Complete progress reports and IEP updates, and participate in CSE meetings as scheduled.
- Develop and coordinate the implementation of individual and classroom behavior plans for managing student behavior to maintain discipline in the classroom; oversee the collecting of behavior data; assist classroom staff with crisis prevention and intervention techniques; assist classroom staff with de-escalation techniques; teach students appropriate behavior strategies.
- Manage and model the implementation of classroom PBIS universal components and Tier 2 and 3 strategies for selected students.
- Schedule and oversee teacher assistants as they assist students in multiple non-instructional environments.
- Provide leadership to assigned teacher assistants in the various clerical and non-instructional duties for your classroom.
- Work collaboratively with other teachers, clinicians, administrators, medical staff and other school staff to provide integrated support, assistance, and treatment to students.
Requirements and Education:
- Bachelor's degree and/or New York State Certification as a Teacher Assistant.
- 2 years experience working with students in an educational setting.
- Certification/Licensure in progress for a Special Education Teacher.

Starting Rate for this role is $20.00/hour

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies' (DSS, OMH, JCAHO, etc.) requirements for elementary aged children.. You will work on-site on our Albany campus and will report to the Residential Supervisor.
Responsibilities:
1:1 Residential Counselor
Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food, cooking, medical care, and providing a reliable and nurturing environment.
You will follow an individual youth’s 1:1 expectations plan, ICMP, and any safety plans in place.
Engage the youth in activities and conversation that are age appropriate and at the appropriate developmental level.
Document in writing a summary of each shift.
Learn the history and specific issues associated with the youth who requires 1:1; will learn how to safely manage any crisis.
Transport 1:1 client for medical, mental health, and other essential needs.
RC1
You will must be able to work within trauma informed framework of program, including identifying and using coping/safety plans, maintaining consistency, providing regular routine, and controlling own response to youth.
Engage and form positive relationship with all youth served.
Complete all mandatory trainings according to programmatic guidelines.
Display basic competency in TCI techniques.
Follow outlined program routines, therapeutic plans of youth, and program rules and protocols.
Work as part of a team including colleague residential counselors, clinicians, on line oncall, supervisors, and other administrative and functional support staff.
Maintain respectful and appropriate communication with internal and external customers, including county workers, medical staff, parents etc.
Complete and maintain documentation to meet programmatic and regulatory standards.
Transport clients for medical, mental health, and other essential needs.

You will facilitate groups/coached visitation, understanding and applying curriculum and appropriate practice models, i.e. DBT, SAY, etc.
Take on specific leadership responsibilities for dorm/cottage, including coordinating activities, participating in program-wide committee.

Form positive relationships with more challenging youth and families.

Display critical thinking and problem solving skills related to residential program needs.

We are seeking a high energy, motivated Point of Sale Warehouse Associate in our Coxsackie, NY facility.
The primary responsibility of the POS Warehouse Associate is to receive, warehouse, distribute and manage POS materials, samples and will-calls. Help Merchandiser in the construction and installation of custom displays. Clean and organize POS warehouse, shop (area where custom displays are constructed) and rebate/shelf talker area.
Manage POS Materials
- Employee handles the receiving of all POS.
- Break down, divide, and organize POS.
- Put away all POS in its designated location of the warehouse.
- Continuously condense POS to make room for new POS.
- Discard expired POS as directed by supervisor.
- Update inventory of all POS on the Data Report and POS System (min 3 times per week)
- Distribute POS to sales reps and Managers or prepare for delivery to accounts as requested. (Place in cubbies or as directed)
- Manage rebates and shelf talkers. Clean and organize this section of the warehouse making sure expiration dates are clearly marked on rebate boxes and expired ones are discarded.
Manage Samples and Will-calls
- Receive and check in samples and will-calls from warehouse or transfer truck.
- Lock up all product in the designated secured area of the warehouse.
- Distribute to the sales reps and managers with proper documentation. (Printed name and signature from recipients as the sample policy states)
Merchandising Responsibilities
- Install both special displays and stock POS by working with merchandisers, customers, and sales management on the coordination of timing and location of displays.
- Daily monitoring of POS System to check for ordered displays awaiting installation.
- Input completed displays into the POS system in a timely manner.
- Maintain a safe and clean work environment by complying with procedures, rules, and regulations including company vans (maintenance) and work area.
Safety Protocol
- Perform daily safety checks on all equipment before use, confirming accordance with specific equipment requirements and maintain these records for 30 days.
- Maintain safety certification on all machinery used on the job. (Forklifts, pallet jacks and cardboard compactor)
- Wear all safety devices required (safety harness, vest and safety shoes)
General Responsibilities
- Receive and distribute incoming packages (FedEx and UPS) to designated recipients.
- Set up and break down salesroom for meetings.
- Help Merchandisers in the construction of custom display pieces.
- Assist in the installation of custom displays when needed.
- Assemble stock POS for distribution (display racks and mass pieces)
- Run deliveries when needed.
Housekeeping/Cleaning
- Clean POS Warehouse (sweep floors and pickup any loose plastic wrap or debris on floor and pallets)
- Clean and organize rebate and shelf talker area of the warehouse.
- Dispose of garbage and expired POS into dumpster.
- Clear and salt walkways at office when needed.
- Maintains a safe and clean working environment by complying with procedures, rules and regulations.
