As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.
Responsibilities:
- Provide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
- Work with family members to identify goals and objectives of treatment
- Provide services based on the needs as identified in the Treatment Plan
- Coordinate with the child and caregiver on the scheduling and provision of services
- Participate in relevant treatment meetings and attend all required trainings
- Complete all documentation
Requirements and Education:
- Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
- Valid NYS Driver's License required

Responsibilities:
- Complete the nursing observations report findings to the registered nurse

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home

Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies' (DSS, OMH, JCAHO, etc.) requirements for elementary aged children.. You will work on-site on our Albany campus and will report to the Residential Supervisor.

Document in writing a summary of each shift.
Learn the history and specific issues associated with the youth who requires 1:1; will learn how to safely manage any crisis.
Seek and obtain training in any particular area specific to the youth who requires 1:1care.
Transport 1:1 client for medical, mental health, and other essential needs.

You will must be able to work within trauma informed framework of program, including identifying and using coping/safety plans, maintaining consistency, providing regular routine, and controlling own response to youth.
Engage and form positive relationship with all youth served.

As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). You will report to the Director of the Behavioral Health Center. Sessions may be conducted in the office or via Tele-mental Health as clinically indicated. This is an on site position
Responsibilities:
- Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
- Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
- Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
- Attend Behavioral Health Center staff meeting twice a month.
- Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
- Provide referrals and linkages to community supports, resources, and services.
Requirements and Education:
- Master's degree in Social Work or Mental Health Counselor.
- Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
- License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. As the Assistant Director of Crisis Residence, you will be responsible for providing support and direction to staff and oversee the day to day operations on site. This position is responsible for monitoring and coordinating intakes to the Crisis Residences. You will manage day-to-day operational needs as oversite to the programs. You will report to the Executive Program Director and will work on-site.
Responsibilities:
- Manage and monitor the referrals for the Crisis Residences and determine eligibility promptly. Coordinate intake appointments for new youth coming into the programs.
- Participate in debriefs for critical incidents. Participate in clinical projects to increase the quality of care.
- Assist Residential Supervisors with creating team-building opportunities and develop staff to provide a cohesive direct care team atmosphere. Supervise a team of Residential Supervisors.
- Facilitate regular supervision to mentor and coach the Residential Supervisors who mentor, develop, train, and manage direct care staff. Help review and revise the orientation of staff to meet the overall objectives of the programs.
- Oversee and assist with necessary Quality Management investigations.
- Complete Admission and Continued Stay Utilization Reviews.
- On Call coverage of Crisis Residences.
- Act as a liaison to community partners to support program service and to the community partners making referrals to the programs.
- Other duties may be assigned.
