POSITION SUMMARY
Northern Rivers Family of Services is seeking a Program Assistant who will maintain smooth and patient-centered care for clients receiving services from the Assertive Community Treatment (ACT) program from intake to discharge.

PROGRAM ASSISTANT – Assertive Community Treatment (ACT)
The ACT Program Assistant is responsible for maintaining smooth and patient-centered care for clients receiving services from the primary licensed Assertive Community Treatment (ACT) program from the referral and intake process through ongoing treatment and discharge.

Essential Duties and Responsibilities of the ACT Program Assistant
- Maintain information of contact information for local first responders and community health agencies.
- Develop and track a staff safety and monitoring system to include staff check-ins, and respond accordingly in case of staff or client incident.
- Alert community partners and referral sources when there are openings in the specialized program.
- Maintain accounting and budget records for recipient and program expenditures.
- Develop and maintain proficiency in using various agency software programs.
- Manage the referral, intake, treatment, and discharge process for the ACT program on a daily basis which includes coordinating appointments and processes with other program staff.
- Work with Health Home to assist in shared information between Health Home and ACT team.
- Management of program staff follow up calls to the discharged participants.
- Compile meeting minutes, general as well as specialized program data and various reports for ACT program stakeholders.
- Responsible for assisting in the development of program budgets, recording expenses, and tracking program spending.
- Schedule Psychiatrist/Psychiatric Nurse Practitioner appointments as directed by Team Lead and ensure that all appointments are correctly scheduled for program and alert clinicians and prescribers when various documentation is required.
- Other duties as assigned.
EDUCATION and/or EXPERIENCE
- Requires an Associate’s degree and three years of related experience, or requisite education and experience, Bachelor’s degree preferred.

POSITION OVERVIEW
Clinical Case Manager, Intensive Aftercare Prevention Program
As a Clinical Case Manager in the Intensive Aftercare Prevention Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.
Responsibilities:
- Provide home and community services to individuals, groups, and families as prescribed in the treatment plan for those enrolled in our Family Preservation Services.
- Educate families in a variety of social-emotional and parenting skills such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc.
- Work in partnership with a Master’s level clinician to provide needed services to an assigned caseload of families.
- Complete and maintain appropriate case records through the completion of intake, assessments, progress notes, uniform case records, reports, etc. following agency, county, and state policy and funding requirements.
- Participate in the comprehensive assessment and creation of the treatment plan for assigned cases to determine and address service needs and ensure the provision of regular treatment plan meetings and service plan reviews.
- Provide liaison contact with collateral resources such as referring agencies, schools, family courts, and other agency personnel serving on behalf of a child and family as it relates to their treatment.
- You will work Monday through Friday from 8:30 am to 5:00 pm with a flexible schedule that includes evenings and a rotating on-call schedule.
Requirements and Education:
- Bachelor's degree in social work, Sociology, Criminal Justice, Psychology, Human Services, or a related field;
- Experience working with at-risk youth and families;
- Home and community-based experience is a plus.

As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). You will report to the Director of the Behavioral Health Center. Sessions may be conducted in the office or via Tele-mental Health as clinically indicated.
Responsibilities:
- Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
- Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
- Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
- Attend Behavioral Health Center staff meeting twice a month.

Requirements and Education:

As a Foster Clinician, you will provide clinical services to children in foster care and support foster families, and families within the foster care system. The Therapeutic Foster Family Program provides foster and pre-adoptive foster homes for abused and neglected children from birth through age 21. Each foster parent receives intensive training and ongoing professional support to promote a positive, successful experience. You will report to the Clinical Supervisor and you will work onsite.
Responsibilities:
- Provide assessment, treatment planning and general counseling for children in foster care.
- Provide individual, group, and family therapy to children and families, when needed
- Conduct specialized risk assessments
- Consult with other staff involved in the care of the child and families
- Attend all case related meetings (Counsel on Special Education, Termination of Parental Rights, staff meetings, service provider meetings, emergency meetings, court), for cases they are serving
- Participate in formal and informal supervision.
- Meet regularly in the community regarding cases they are seeing for general counseling.
- Have regular communication/meeting with birth families around progress/problems of children in care and how to support them in and work towards permanency.
- Provide regular case documentation for all children and families on workload including assessments, progress notes, Family Service Plan, etc.
Requirements and Education:
- Requires a Master’s degree in counseling with related experience.

- Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
- Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
- Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
- Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
- Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).
Shift/Schedule:

As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicated to individuals, families and groups in a Permanency Resource Center. As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. - Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
- Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
