POSITION OVERVIEW

As a Family Support Worker, you will work as part of the Short-term Services Program which was developed as an intensive case-management service to rapidly resolve the concrete and referral needs of families entering the Child Welfare System. By successfully resolving these issues within 90 days of admission, families are at a reduced risk for being referred to Long-term CPS. Services are provided in the office, home, school, and community. You will report to the Coordinator of Short-term Services and will work onsite with community-based travel required.

Responsibilities:

• Manage a full caseload.
• Complete all required trainings, participate in supervision with supervisor and attend required meetings and in-services as directed by supervisor.
• Establish positive and professional relationships with families.
• Model and demonstrate problem-solving and decision-making skills.
• Work with SCDSS case managers to identify service needs of at-risk families.
• Have a thorough knowledge of community services in Schenectady County.
• Help families obtain services they need by making referrals, providing transportation, accompanying them to appointments, assisting with applications (medical, schools, DSS, etc.).
• Maintain good working relationships with DSS/Support Unit caseworkers.
• Complete progress notes or other case record notes promptly.
• Maintain cultural sensitivity, competence and client confidentiality.
• Other duties as assigned.

Requirements and Education:

• Requires an Associate’s degree and one to two years of related experience, or equivalent education and experience.
• Must have a valid NYS driver’s license.

Home Based Crisis Intervention Specialist

As a Home Based Crisis Intervention (HBCI) Specialist, you will support families and youth in crisis by providing in-home services. You will assist families in implementing behavior management techniques and crisis de-escalation skills with their children and support children in appropriately communicating their needs.You will report to the Senior Director HBCI and work onsite with community-based travel required.

The HBCI team provides short-term (up to 6 weeks), intensive intervention to youth and families who are in crisis. We work with parents and caregivers to help them help their children. The short-term format means the work is fast-paced and outcome oriented, which means that the HBCI team will produce meaningful outcomes in a short period of time.

Responsibilities:

• Respond to the needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling.
• Promote safety of the child, family, and community in the delivery of crisis intervention services.
• Tailor services and flexibly schedule sessions based on family member’s needs.
• Conduct comprehensive assessment of family strengths, values, skills, problems, needs, barriers to goal attainment and collaborate in goal setting and service planning.
• Use a variety of teaching methods to help family members acquire, maintain and generalize skills.
• Advocate for and provide concrete services and supports and teach family members to meet their concrete needs.
• Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves. Prepare family for transition and service closure.
• Provide consultation to intra-agency service providers and to community service providers. Maintain appropriate liaisons and relationships with external and internal providers, particularly as this relates to County Mental Health Departments working with the HBCI. Fulfill all program-reporting requirements.
• Participate in the Continuous Quality Improvement process and provide leadership in the collection of data and the formulation of recommendations and conclusions as assigned.

Requirements and Education:

• Bachelor’s degree in human services, or Master’s degree in Social Work or Mental Health Counseling and at least two years of experience in a home-based and/or crisis intervention services program.
• LMSW or LMHC licensure preferred.
• Experience in the Homebuilder model of crisis intervention is preferred.
• Must have a valid NYS driver's license.

Clinician, Families Work

Licensed Social Workers or Mental Health professionals are eligible for a $7500 signing bonus!

As a Clinician in the Families Work program, you will maintain a caseload of five families and meet with each weekly. Families Work provides six to nine months of intensive services to families whose children either are at risk of being removed from their homes or are already in placement. Families Work also facilitates parent education groups using Incredible Years and Active Parenting curricula. The goals of the program are to prevent out-of-home placement for children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and work in a combination of on-site and remote with travel to client's homes required within Columbia County. You will work Monday through Friday with a flexible schedule, where you work independently but with a supportive team approach.

Responsibilities:.

• Provide in-home trauma-informedservicesincluding cognitive behavioral and family therapy to address parent/child issues; individual and couples therapy; parenting skills and child development training, development and monitoring of behavioral plans in the home and at school; and skill-building services including anger management, stress management, problem solving, and coping skills
• Provide parent education with parents/ legal guardians enrolled in Parent Education services and ensure the coordination of virtual and in-person parent education classes of various age ranges, around the clients’ schedules
• Provide home and community-based consultation/ intervention with identified parents/ legal guardians to reinforce core concepts of the evidence-based curriculum and help ensure parental comprehension and integration of the concepts
• Provide support services including transportation, budgeting, housing and household management, case planning, on-call crisis support, and school consultations
• For children who are in placement, work with parents and treatment team to discharge children to their permanency goal as quickly and safely possible and/ or to assist families with meeting needs at a lower level of care
• Complete necessary all records and client information in accordance with agency, county, and state policy and funding requirements

Requirements and Education:

• Master's degree in social work, mental health counseling, or equivalent from a graduate school program
• 0-2 years post-masters experience

Clinician, Intensive Aftercare Prevention Program

Licensed Social Workers or Mental Health professionals are eligible for a $7500 signing bonus!

As a Clinician in the Intensive Aftercare Prevention program, you will provide home-based clinical services to children and their families to help preserve or reunite them following Northeast Parent & Child Society's stated purpose and mission of protecting children, preserving families, and strengthening communities. The Intensive Aftercare Prevention Program (IAPP) is a home and community-based program assisting families who have children in foster care or children who are at risk of being placed in foster care. You will report to the Program Coordinator and you will work remotely with travel to client homes required.

Responsibilities:

• Provide home and community-based assessment, planning, and treatment with individuals, groups, and families with those enrolled in our Family Preservation services (the level of intensity/number of direct service hours per family is dependent upon the services in which one is employed)
• Complete necessary all records and client information following agency, county, and state policy and funding requirements
• Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
• Responsible for the clinician decisions when assigned and paired to work as a team with a Bachelors Clinician (Family Case Manager or Clinician Case Manager).
• Ensure provisions of regular treatment plan meetings and services plan reviews.

Requirements and Education:

• Requires a Masters degree in Social Work, Mental Health Counseling or related field
• Minimum of one year of post-graduate experience.
• License Preferred: LMSW, LCSW, LMHC, or LMFT
• Must have a valid driver's license.

Clinical Case Manager, Prevention Services

As a Clinical Case Manager in the Prevention Services program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.This position covers Saratoga County.

Responsibilities:

• Develop treatment plans with families to help identify and meet their goals.
• Develop a psychosocial assessment. Connect with the youth's family to monitor progress and support the family's effort to keep the youth safe.
• Coordinate services with service providers and collaterals involved with the youth and their family.
• Complete paperwork within agency, county, and state guidelines.
• Ability and willingness to do the job as changes arise.

Requirements and Education:

• Bachelor's degree in social work, sociology, criminal justice, psychology, human services or related field;
• Experience working with at-risk youth and families;
• Home and community-based experience a plus.
