POSITION OVERVIEW
Clinical Case Manager, Prevention Services
As a Clinical Case Manager in the Prevention Services program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.This position covers Saratoga County.
Responsibilities:
- Develop treatment plans with families to help identify and meet their goals.
- Develop a psychosocial assessment. Connect with the youth's family to monitor progress and support the family's effort to keep the youth safe.
- Coordinate services with service providers and collaterals involved with the youth and their family.
- Complete paperwork within agency, county, and state guidelines.
- Ability and willingness to do the job as changes arise.
Requirements and Education:
- Bachelor's degree in social work, sociology, criminal justice, psychology, human services or related field;
- Experience working with at-risk youth and families;
- Home and community-based experience a plus.

As a Mobile Clinical Site Supervisor, you will oversee daily management and oversight of all staff to ensure management of the crisis line and onsite response to all 6 counties is coordinated. You will report to the Director of Crisis Services and you will work on-site.
Responsibilities:
- Provide daily management and oversight to staff including:
- Manage the crisis line.
- Monitor triages to ensure appropriate call management response.
- Assist with coordinating team response based on staffing per call.
- Maintain knowledge of the schedule for team response.
- Monitor Teams Chat and provide supervision/coordination.

- Enhance collaborative teamwork between all staff.
- Cross shifts with evening hours expected to provide support to both shifts across the hours of 7-11pm for Mobile Crisis Services; evening hour coverage responsibility shared with another site supervisor.
Requirements and Education:
- MSW/MHC required
- 3 - 4 year's experience
- LCSW required; may consider LMHC
- Valid NYS Driver's License

The Youth Assertive Community Treatment (ACT) program is seeking a Psychiatric Nurse Practitioner. This position is responsible for providing comprehensive Psychiatric services to adults and youth who are enrolled in our outpatient clinic program and a 28-person caseload of youth enrolled in our Youth ACT program. This position is full-time, 40 hours per week. Must be able to seechildren.
Responsibilities:
Provide comprehensive psychiatric services to youth enrolled in Youth ACT and adults and youth enrolled in our BehavioralHealth Clinic as assigned
Provide consultation to staff on diagnostic and treatment questions
Participate in formal and informal training and education to staff
Prescribe medications, monitor, and review of drug regimen for enrolled individuals
Participate in quality assurance activities, consult with the Youth ACT Team in developing and reviewing service plans for all Youth ACT enrolled individuals
Provide clinical expertise, consultation, and technical assistance to the Youth ACT Team/Behavioral Health, cooperate with the County and participate in any quality audit and utilization review
Provide on-site/community based/home visits medication evaluations and reviews for Youth ACT enrolled participants.
Education and/or Experience:
Requires Board Certification by the American Board of Psychiatry and Neurology. Must have a minimum of one year experience working with youth and adults diagnosed with mental illness.
Must be fully licensed Nurse Practitionerin the State of New York.

Per Diem Clinician,Permanency Resource Center
As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicatedto individuals, families and groups in a Permanency Resource Center.This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases. Therapy is to be provided in the community or in office, and is paid for by the program, not through insurance reimbursement. You will report to the Licensed Clinician and you will work in a hybrid model with community-based travel required.
Responsibilities:
- Provision of outpatient therapy remotely or in person with youth or caregivers within the Permanency Resource Center system.
- Document the contact within the Evolv system.
- Attend and participate in individual supervision on at least a monthly basis and more frequently if the acuity of the assigned cases requires this.
- Provide therapeutic services to individuals assigned to case load.
- Provide case management services as clinically indicated.
- Provide referral, collateral and linkage services.
- Record and maintain case records according to program standards.

Clinician, Family Treatment Court
As a Clinician, you will provide home and community-based clinical services with Schenectady Family Treatment Court participants and their family members to help preserve or reunite them in accordance with Northern Rivers Family of Service's stated purpose and mission of protecting children, preserving families and strengthening communities. You will report to the Director of Home and Community-based Services and will work onsite with community-based travel required.
Responsibilities:
- Conduct comprehensive evaluations within agreed upon timelines in-person or via Telehealth of parents and children whose cases are accepted into the Family Treatment Court.
- Make referrals for recommended mental health treatment, perform ongoing case management, and report back to the Family Treatment Court team on the progress of each family member who is receiving mental health services. For individuals not currently working with a provider, partner with a licensed provider and consult with any other relevant providers to whom those individuals are referred to services.
- Work with substance use disorder treatment providers to ensure that the treatment plan for addressing any identified mental health issues works in conjunction with the treatment plan for addressing the substance use disorder issues.
- Participate in regular team meetings.
- Provide strategies to parents whose mental health issues create safety concerns for their children.
- Provide knowledge and resources to the treatment court team, as well as to parents, regarding children who have mental health diagnoses.
- Maintain documentation (such as project logs, reports and records in appropriate files and databases) and provide continuous feedback to Family Treatment Court team members regarding participant progress.
- Assist in collecting and maintaining data as required by Office of Juvenile Justice and Delinquency Prevention.
- Other duties as assigned.
Requirements and Education:
- Requires a Master’s degree in Social Work, Mental Health Counseling or related filed and at least one year of post-graduate experience.

As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.
Responsibilities:
Coordinates, monitors, and documents the health care of foster children placed within the foster care program, including but are not limited to:
Tracking health and dental exams.
Scheduling and assisting with psychiatric med reviews for children in the FC program
Reviews and documents medical and dental reports and coordinates follow-up as directed
Coordinates referrals made to specialty clinics and follows up on recommended tests, treatments, or therapies
Follows up regarding emergency room visits and sick visits to ensure that the prescribed medical regimen is being carried out
On a case by case basis, completes or coordinates completion of forms required for specific purposes, i.e. school, working papers, or camp
Monitors both medical and dental compliance and notifies foster parents and foster care case manager when compliance is in jeopardy
Reviews completed examinations, medical consults, and med-ex forms for foster care children and electronically stores as an attachment into chart
Serves as a liaison between internal and external medical service consultants, foster care supervisor, and foster care case managers
Serves as a resource to program staff concerning the health status of children in the foster care program, specifically those children with chronic health needs
Participates in service planning and treatment reviews and clinical meetings as indicated
Maintains a close working relationship with community-based health care facilities. On a regular basis seek consults and lab work for identified children in the foster care program, reviews bills to ensure accuracy related to service provision, discusses and resolves any problems and obstacles to children accessing appropriate care
Travel to foster parent homes with the occasional transport is required for this position

Educate service providers regarding an individual's role in developing a crisis plan for themselves and in crisis prevention. Ensure that the individual is an integral part of the crisis planning process.
Inform service providers (including Mobile Crisis and PCFC providers) regarding person- centered and strength-based practices. Maintain and model a focus on strengths.
Advise Clinical Supervisor.
Ensure that clients are satisfied with the services they are receiving and, if not, help them advocate for what they need.
Educate individuals and primary caretakers on how to advocate if services are not meeting their needs.
Be the liaison to other local supports in the community.
Link individuals to Peer Support Services activities such as support groups, social events, newsletter.
