Description
As a Family Advocate in the mobile crisis department you will be a member of Mobile Crisis Services. You will assist the clinicians on Mobile visits by providing support and advocacy for family members of the child or adolescent in crisis. You will be a resource to the program by sharing knowledge of common challenges families have experienced with a mental health diagnosis or other identified disabilities and sharing knowledge about communities resources. You will report to the Clinical Site Supervisor and work onsite.
Responsibilities:
- Respond to onsite crisis call with clinicians to assist with crisis planning and stabilization.
- Help with follow-up calls to families, on the crisis call and telephonically
- Collaborate with service providers involved with the children/adolescents and their families.
- Provide information and peer to peer support to families who are served by the program
- Educate and empower families regarding their roles in developing crisis plans and crisis prevention with their children/adolescents.
- Ensure families are satisfied with the services they are receiving and, if not, help them advocate for what they need and how to advocate.
- Be a liaison to other local supports in the community.
- Function within a diverse treatment team and maintain work relationships with team members, agency colleagues, external providers and clients (respect amongst disciplines, trauma-informed practice)
- Understand the program mission and expectations of the county funding sources
- Remain in compliance with mandatory agency and program trainings and additional trainings, as assigned
- Use clinical team members for support regarding client contact
- Attend all regular staff meetings
- Participate in supervision and annual job performance evaluation
Requirements and Education:
- High School Diploma or GED required.

Inform service providers (including Mobile Crisis and PCFC providers) regarding person- centered and strength-based practices. Maintain and model a focus on strengths.
Advise Clinical Supervisor.
Ensure that clients are satisfied with the services they are receiving and, if not, help them advocate for what they need.
Educate individuals and primary caretakers on how to advocate if services are not meeting their needs.

POSITION SUMMARY
Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- At minimum, 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan.
- Provide services based on the youths needs and preferences as identified in the Treatment Plan.
- Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.

- Staff need to maintain consistency, provide regular routines and be aware of their own emotions when dealing with the youth in care.
- Engage and form positive relationships with all youth served.
- Maintain respectful and appropriate communication with internal and external customers, including county workers, medical staff, parents etc.
- Develop and maintain relationships with community organizations and consumers to expand opportunities for community connections.
- Facilitate service provision and attend relevant meetings as required.
- Provide information to collateral contacts for the purpose of Treatment planning and assist in tracking progress as required.
- Attend all required trainings and meet regularly for supervision.
- Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with case manager to find resources in the community for educational and vocational skills and programming for discharge.

English/Special Education Teacher
**Sign On Bonus $5,000** (paid in 2 parts 1/2 at 30 days and 1/2 at a year)**
Available shifts - Monday - Friday between 8:00 am - 4:00 pm - no weekends
As an English and Special Education Teacher, you will develop and implement classroom instructional plans for students and support student success in academic and behavioral goals. You will report to the Assistant Principal and you will work onsite.
Responsibilities:
- Follow all Agency policies and procedures, along with state and federal regulations about our school and students.
- Provide individualized instruction in English to high school students in accordance with New York State curriculum standards and graduation requirements.
- Develop and implement Individualized Educational Plans (IEPs) for all students on your caseload. Ensure that accommodations and modifications are provided. Provide specially designed instruction and monitor student progress toward academic and behavioral goals. Complete progress reports and IEP updates, and participate in CSE meetings as scheduled.
- Coordinate the creation of special strategies for reinforcing material or skills based on an understanding of individual students, their needs, interests, and abilities; create and modify materials to match student abilities.
- Develop and coordinate the implementation of single and classroom behavior plans for managing student behavior to maintain discipline in the classroom; oversee the collecting of behavior data; assist classroom staff with crisis prevention and intervention techniques; assist classroom staff with de-escalation techniques; teach students appropriate behavior strategies.

- Requires a Bachelor's degree in Special Education (Master's Degree preferred) or subject area, and ability to work with students who demonstrate serious emotional and behavioral challenges, ages 6-21.
