- Maintain a caseload of 10 – 12 clients and meet service needs within a six to eight week time frame.
- Receive referrals and collaborate with referral source, as needed.
- Rapidly engage clients and complete intake paperwork including the Social Determinants of Health questionnaire.
- Effectively interface with government offices, social service providers and other community resources to actively link clients to services, assist in their application for benefits, and to obtain concrete goods.
- Maintain positive communication with clients and provide active assistance including transportation, attending meetings, complete applications, etc.
- Advocate for client with providers, as well as troubleshoot barriers that impede or otherwise negatively impact the ability to access services/resources.
- Assist clients by providing education, information and support.
- Complete documentation and data collection, as needed.
- Become familiar with government benefits, community and neighborhood resources, etc.
- Participate in and contribute to team meetings. Attend relevant program development activities, trainings, and conferences.

The incumbent for this position will provide employment services to assist individuals who are searching for career opportunities, or may be looking for support in their current roles.
- Supports the development and customization of resumes, cover letters, and thank you letters.
- Work with participants to develop and hone interviewing skills (including disclosing disabilities and discussing accommodations if applicable) with employers. Develop a placement or job-searching plan in accordance with the participant’s vocational interests and abilities.
- Assess and assists participants with any needed job-seeking skills including, but not limited to, locating employment opportunities and supporting them to complete job applications. When possible, this role will provide individuals with interview and general presentation feedback gained from prospective employers. Offer skills development coaching as needed.

- Develop a monthly recreation calendar and news letter for the Golden Club and develop a weekly supply list so to obtain and pick up donations.
- Work with the team to plan activities, weekly outings and find social activities in the community.
- Networking within the community and communicate with other providers to ensure the best interest of each member.

As a Case Manager, you will work with the children in care, the Department of Social Services, foster families, and birth families in the journey toward achieving permanency in multiple settings. You will report to the Program Coordinator and work on-site with some travel.

As a Clinician in the Intensive Aftercare Prevention program, you will provide home-based clinical services to children and their families to help preserve or reunite them following Northeast Parent & Child Society's stated purpose and mission of protecting children, preserving families, and strengthen communities. - Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed)

- Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
