The Youth Assertive Community Treatment (ACT) program is seeking a Psychiatric Nurse Practitioner. This position is responsible for providing comprehensive Psychiatric services to adults and youth who are enrolled in our outpatient clinic program and a 28-person caseload of youth enrolled in our Youth ACT program. This position is full-time, 40 hours per week. Must be able to seechildren.
Responsibilities:
Provide comprehensive psychiatric services to youth enrolled in Youth ACT and adults and youth enrolled in our BehavioralHealth Clinic as assigned
Provide consultation to staff on diagnostic and treatment questions
Participate in formal and informal training and education to staff
Prescribe medications, monitor, and review of drug regimen for enrolled individuals
Participate in quality assurance activities, consult with the Youth ACT Team in developing and reviewing service plans for all Youth ACT enrolled individuals
Provide clinical expertise, consultation, and technical assistance to the Youth ACT Team/Behavioral Health, cooperate with the County and participate in any quality audit and utilization review
Provide on-site/community based/home visits medication evaluations and reviews for Youth ACT enrolled participants.
Education and/or Experience:
Requires Board Certification by the American Board of Psychiatry and Neurology. Must have a minimum of one year experience working with youth and adults diagnosed with mental illness.
Must be fully licensed Nurse Practitionerin the State of New York.
Must be able to work independently

National Provider Identification (NPI) - issued by the federal Center for Medicare and Medicaid Services (CMS). All electronic prescriptions in NYS require an NPI. Claims processing functions also require the NPI of nurse practitioners and physicians.
Federal Drug Enforcement Administration (DEA) number– issued by the United States Department of Justice Department DEA to nurse practitioners and physicians to prescribe controlled substances. DEA registration must be renewed every three years for those issued a prescriber registration.
NYS Bureau of Narcotic Enforcement (NYS DOH BNE) registration – issued by the NYS DOH BNE to nurse practitioners and physicians to prescribe controlled substances. NYS DOH BNE registration must be renewed every two years for those issued a prescriber registration.
NYS Medicaid Enrollment - in cases in which a Prescriber is prescribing to a NYS Medicaid enrolled client. Revalidation must occur at least every five years.

Description
Clinician, Family Treatment Court
As a Clinician, you will provide home and community-based clinical services with Schenectady Family Treatment Court participants and their family members to help preserve or reunite them in accordance with Northern Rivers Family of Service's stated purpose and mission of protecting children, preserving families and strengthening communities. You will report to the Director of Home and Community-based Services and will work onsite with community-based travel required.
Responsibilities:
- Conduct comprehensive evaluations within agreed upon timelines in-person or via Telehealth of parents and children whose cases are accepted into the Family Treatment Court.
- Make referrals for recommended mental health treatment, perform ongoing case management, and report back to the Family Treatment Court team on the progress of each family member who is receiving mental health services. For individuals not currently working with a provider, partner with a licensed provider and consult with any other relevant providers to whom those individuals are referred to services.
- Work with substance use disorder treatment providers to ensure that the treatment plan for addressing any identified mental health issues works in conjunction with the treatment plan for addressing the substance use disorder issues.
- Participate in regular team meetings.
- Provide strategies to parents whose mental health issues create safety concerns for their children.
- Provide knowledge and resources to the treatment court team, as well as to parents, regarding children who have mental health diagnoses.
- Maintain documentation (such as project logs, reports and records in appropriate files and databases) and provide continuous feedback to Family Treatment Court team members regarding participant progress.
- Assist in collecting and maintaining data as required by Office of Juvenile Justice and Delinquency Prevention.
- Other duties as assigned.

- Requires a Master’s degree in Social Work, Mental Health Counseling or related filed and at least one year of post-graduate experience.

Per Diem Clinician,Permanency Resource Center
As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicatedto individuals, families and groups in a Permanency Resource Center.This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases.
