As an Employment Counselor, you will provide employment services to assist individuals searching for career opportunities or may be looking for support in their current roles. You will report to the Supervisor of Prevocational and Senior Services and you will work on-site with traveling required.

Responsibilities:

• Support the development and customization of resumes, cover letters, and thank you letters.
• Work with participants to develop and hone interviewing skills (including disclosing disabilities and discussing accommodations if applicable) with employers.
• Assess and assist participants with any needed job-seeking skills including locating employment opportunities and supporting them to complete job applications. When possible, you will provide individuals with interview and general presentation feedback gained from prospective employers. Offer skills development coaching as needed.
• Provide one-on-one guidance to participants with a focus on aligning interests, strengths, and capabilities with employment opportunities.
• Develop a placement or job-searching plan in accordance with the participant’s vocational interests and abilities.
• Network with employment site leadership team members.
• Work with participants to address any transportation challenges that may be present.
• Complete case notes.
• Complete monthly reports.

Requirements and Education:

• Bachelor’s degree (BA/BS) or 4 years of equivalent experience.

As a Peer Support Specialist, you will be a member of the Mobile Team, assisting the clinicians on Mobile visits and providing support and advocacy to individuals in crisis. You will also be a resource to the clinicians and case manager, through sharing their knowledge of common challenges individuals with SPMI endure and sharing knowledge about community resources. You will be an individual (adult) with lived experience with mental illness. You will report to the Executive Program Director and you will work on-site.

• Collaborate with service providers involved with individuals with SPMI.

• Educate and empower clients and their supports regarding their roles in developing crisis plans and in crisis prevention.

• Educate service providers regarding an individual's role in developing a crisis plan for themselves and in crisis prevention. Ensure that the individual is an integral part of the crisis planning process.

• Inform service providers (including Mobile Crisis and PCFC providers) regarding person- centered and strength-based practices. Maintain and model a focus on strengths.

• Advise Clinical Supervisor.

• Ensure that clients are satisfied with the services they are receiving and, if not, help them advocate for what they need.

• Educate individuals and primary caretakers on how to advocate if services are not meeting their needs.

• Be the liaison to other local supports in the community.

Clinician, School Based - Days

MSW/MHC's with a Limited Permit are eligible for a $2,500 signing bonus!

As a Clinician, you will work collaboratively with the school district and as part of an interdisciplinary team with identified students, by providing clinical counseling to the youth and their family in an outpatient mental health clinic, licensed by the New York State Office of Mental Health (OMH), imbedded within the School Building. You will report to the Director of the Behavioral Health Center and you will work onsite.

Responsibilities:

• Work collaboratively with the school district to identify and treat a caseload of students with mental health needs providing a comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
• Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation and assessments as indicated.
• Collaborate with the associated clinic prescriber, other medical and mental health providers, school staff and any other community supports for client care.

Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• At minimum, 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan.

Clinician, School Based - Days

MSW/MHC's with a Limited Permit are eligible for a $2,500 signing bonus!

As a Clinician, you will work collaboratively with the school district and as part of an interdisciplinary team with identified students, by providing clinical counseling to the youth and their family in an outpatient mental health clinic, licensed by the New York State Office of Mental Health (OMH), imbedded within the School Building. As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. You will provide these interventions by telephone and on-site mobile visits and will implement them using a strength-based, trauma-informed, systems-oriented perspective, including the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance. You will work on-site with travel to the community and individuals' homes.

Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

As a Clinical Case Manager in the Intensive Aftercare Prevention Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.

• Provide home and community services to individuals, groups, and families as prescribed in the treatment plan for those enrolled in our Family Preservation Services.
• Educate families in a variety of social-emotional and parenting skills such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc.
• Work in partnership with a Master’s level clinician to provide needed services to an assigned caseload of families.

Responsibilities:

• Oversee and ensure implementation of all applicable emergency systems, agency policies, and program procedures.
• Respond to crises within assigned Residential Program/locations. Ensure response follows agency policy and protocol appropriate to the situation, and ensure that the individual clients behavior management plan(s) are followed. Ensure any physical intervention is conducted following TCI standards. Ensure completion of all forms and appropriate documentation of every restraint.
• Provide support to teams when asked for behavioral interventions and events. Will direct the team in absence of residential supervisor and will be a source of training , supporting and modeling for other staff.
• Ensure the security of campus and community locations, responding to any security concerns. Conduct random checks of assigned locations to ensure safety of residents.
• Be the primary person in charge of calling the police for any resident considered missing, completing or ensuring completion of the Missing Person Report form and canceling Missing Person Reports upon a residents return.
• Ensure contacts have been made with the residents family and guardian for all significant incidents.
• Maintain regular entries in the Online/On call log regarding all significant events that occur, and provide the leadership with specific additional information as. Ensure completion of documentation for all significant incidents.
• Ensure significant incidents are called into the Justice Center as applicable.
• Serve as the central person in charge of off campus trips. Approve and log all off-campus trips and assure such trips have proper staff coverage.
• Oversee and transport clients for medical, mental health, and other essential needs.
• Check and assure that all visitors have prior approval to be on the grounds.
• Be available to hear and document any neighbor concerns.

Requirements and Education:

• Requires an associate's degree;bachelor's degree and equivalent experience preferred.
• 3 years of experience, or equivalent education and experience.
• Drivers license is required.
• Must be able to pass the required physical.
• Must be able to complete and pass TCI training.
