POSITION SUMMARY

Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• At minimum, 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan.
• Provide services based on the youths needs and preferences as identified in the Treatment Plan.
• Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
• Complete all required documentation and submit to the Supervisor within required timeframes.
• Treatment plan goals may include Independent Living or social skills training examples Career Planning, Self-Care, Daily Living, Housing, Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures and using their coping skills when feeling dysregulated.
• Staff need to maintain consistency, provide regular routines and be aware of their own emotions when dealing with the youth in care.
• Engage and form positive relationships with all youth served.
• Maintain respectful and appropriate communication with internal and external customers, including county workers, medical staff, parents etc.
• Develop and maintain relationships with community organizations and consumers to expand opportunities for community connections.
• Facilitate service provision and attend relevant meetings as required.
• Provide information to collateral contacts for the purpose of Treatment planning and assist in tracking progress as required.
• Attend all required trainings and meet regularly for supervision.
• Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with case manager to find resources in the community for educational and vocational skills and programming for discharge.

EDUCATION and/or EXPERIENCE

• High School Diploma/GED required
• At least 2 years of experience

CERTIFICATION / LICENSURE

• Valid drivers license required

Full-time Licensed Social Workers are eligible for a $7,500 retention bonus!

As a Clinician, you will coordinate client assessments and work with the child, family, legal guardian, law guardian, psychiatrist, nurse, psychologist, educators, child care staff, and other providers to prepare, implement and monitor a comprehensive, permanency-focused case plan for children in the Residential Program. You will report to the Director of the Residential Treatment Facility and will work on-site.

Responsibilities:

• Work with the direct treatment providers to establish treatment goals for children, utilizing historical and current available information.
• Meet and communicate with biological families, guardians, foster families, and case workers.
• Assess individual and family situations.
• Identify problem areas, treatment goals and methods to attain goals.
• Provide therapeutic services to children on at least a weekly basis and families on at least a bi-weekly basis using differential treatment modalities.
• Work with interdisciplinary team to develop and implement a comprehensive treatment plan and to monitor ongoing progress of child and family.
• Learn and practice case coordination skills.
• Work with referring and collateral agencies and prepare reports and correspondence as needed.
• Record and maintain case record in accord with Agency, OMH, and other state mandates.
• Participate in intake assessments as assigned.
• Diligently plan for discharge/permanency, at the time of admission.
• Actively communicate with clients and customers through phone and email correspondence.
• Ensure quality oversight and improvement services.

Requirements and Education:

• Requires a Master’s degree; related experience preferred.
• RTF Clinician requires two years of related experience.
• Professional licensure required: (LMSW with 3 years experience, LMHC-D, LMFT-D, LCAT, LCSW)
• Valid NYS driver's license required
• Must pass physical.
• Must be able to complete TCI training.

As a Special Education Teacher, you will develop and implement classroom instructional plans for students and support students success in academic and behavioral goals. You will report to the Assistant Principle and you will work onsite.

Responsibilities:

• Follow all Agency policies and procedures, along with state and federal regulations about our school and students.
• Develop and implement a classroom instructional plan based on a students IEP and grade and subject area curricula; work with content area teachers to ensure that their lessons are differentiated as to meet the needs of all students.
• Develop and implement Individualized Educational Plans (IEPs) for all students on your caseload. Ensure that accommodations and modifications are provided. Provide specially designed instruction and monitor student progress toward academic and behavioral goals. Complete progress reports and IEP updates, and participate in CSE meetings as scheduled.
• Coordinate the creation of special strategies for reinforcing material or skills based on an understanding of individual students, their needs, interests, and abilities; create and modify materials to match student abilities.
• Develop and coordinate the implementation of single and classroom behavior plans for managing student behavior to maintain discipline in the classroom; oversee the collecting of behavior data; assist classroom staff with crisis prevention and intervention techniques; assist classroom staff with de-escalation techniques; teach students appropriate behavior strategies.

Requirements and Education:

• Requires a Bachelor's degree in Special Education (Master's Degree preferred) or subject area, and ability to work with students who demonstrate serious emotional and behavioral challenges, ages 6-21. General education certification with enrollment in a program that leads to special education certification may be considered.
• Requires that you acquire a NYS certification in Special Education or appropriate subject area, or enrollment in a program that leads to NYS teacher certification within one year of hire

As a Registered Nurse, you will provide nursing care and services to a youth and adolescent population with emotional, behavioral and physical needs using the nursing process (assessing, diagnosing, planning, implementing, and evaluating). You will report to the Nursing Manager and you will work onsite.

Responsibilities:

• Complete the nursing assessment, evaluation and treatment plan of care for the physical and behavioral/emotional health of the residents
• Case management of outside provider visits (medical and dental) including annual assessments
• Review lab and test results, reporting critical values to providers
• Medication management including ordering medications and obtaining prior authorizations
• Dispense medications; report/discuss observed side effects or other concerns with consulting psychiatrist
• Provide client and family education regarding medications and treatments\
• Collaborate with our staff regarding concerns and behaviors
• Communicate with medical director/providers regarding changes in client’s health/behavior
• Participate in Interdisciplinary Medication Review meetings
• Nursing and medical documentation using EHR
• Provide training to residential counselor staff regarding reporting/recording of observedbehaviors and possible side effects relating to medications

As an Online/Oncall, you will ensure the security of campus and community residential locations, including working with staff, maintaining communication with resident’s family and guardians, and overseeing resident transportation needs. You will report to the Online/Oncall supervisor and you will work on-site.

Responsibilities:

• Oversee and ensure implementation of all applicable emergency systems, agency policies, and program procedures.
• Respond to crises within assigned Residential Program/locations. Ensure response follows agency policy and protocol appropriate to the situation, and ensure that the individual clients behavior management plan(s) are followed. Ensure any physical intervention is conducted following TCI standards. Ensure completion of all forms and appropriate documentation of every restraint.
• Provide support to teams when asked for behavioral interventions and events. Will direct the team in absence of residential supervisor and will be a source of training , supporting and modeling for other staff.
• Ensure the security of campus and community locations, responding to any security concerns. Conduct random checks of assigned locations to ensure safety of residents.
• Be the primary person in charge of calling the police for any resident considered missing, completing or ensuring completion of the Missing Person Report form and canceling Missing Person Reports upon a residents return.
• Ensure contacts have been made with the residents family and guardian for all significant incidents.
• Maintain regular entries in the Online/On call log regarding all significant events that occur, and provide the leadership with specific additional information as. Ensure completion of documentation for all significant incidents.
• Ensure significant incidents are called into the Justice Center as applicable.
• Serve as the central person in charge of off campus trips. Approve and log all off-campus trips and assure such trips have proper staff coverage.
• Oversee and transport clients for medical, mental health, and other essential needs.
• Check and assure that all visitors have prior approval to be on the grounds.
• Be available to hear and document any neighbor concerns.

Requirements and Education:

• Requires an Associate’s degree plus three years of experience, Bachelor’s degree preferred, OR HS plus at least five years of experience required.
• Drivers license is required.
• Must be able to pass the required physical.
• Must be able to complete and pass TCI training.

As a Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. You will report to the Case Manager Supervisor. You will work in a hybrid environment with some travel to client's homes in the community.

Responsibilities:

• Function as a systems navigator to families who have adopted or who have children in guardianship or kinship care through the provision of case management.
• Meet with families in their home to assess needs
  • Assess needs, conduct service planning, service plan implementation, service coordination, monitoring and follow-up, reassessment and review with the family via in-home or community monthly visits. Services include but are not limited to information and referral services; referral to legal services; attendance at family court dates; transportation; housing assistance; childcare; basic family needs (vital needs in emergencies and with expenses that are unforeseen); educational advocacy services; and facilitating appointments with social services, medical health providers, and mental health services.
• Respond to the request for in home support referrals for resources and services
• Provide crisis intervention supports when needed
• Work with family to access needed respite services
• Conduct follow up contacts with families at prescribed intervals
• Participate in family engagement activities
• Co-lead support groups
• Participate in training programs offered by the program as assigned
• Use lived experience to empathize and provide hope. Preference for someone with experience with kinship, adoption or guardianship as a youth or in a parenting role.
• Maintain records including required data collection information according to the program standards and expectations. Case Managers must maintain Evolv record with progress notes and all pertinent documentation, a discharge summary and post-case closure documentation.
• Participate as a trainer in programs in the community, to increase understanding of the needs of families who have adopted or have children in guardianship or kinship care
• Complete risk assessments as required by Northern Rivers Quality Management Department

Requirements and Education:

• Bachelor’s level degree in social work, psychology, or human services, or a bachelor’s level degree in any field with at least two years of case management experience; or an associate’s level degree in psychology or human services with at least two years of case management experience.
• Must be able to communicate effectively and develop positive working relationships
• A valid NYS drivers license
