As a Licensed Clinical Community Service Providers, you will provide Children and Family Treatment Support Services (CFTSS). These mental health services available with New York State Children's Medicaid program, give children (up to age 21) and their families the power to improve their health, well-being and quality of life. This program is intended to offer opportunities to better meet the behavioral health needs of children early on and to prevent the onset or progression of behavioral health conditions. These services strengthen children and their families, and help them to make informed decisions about their care and are provided either at home or in the community. Responsibilities
- Provide Home and Community-based Assessments, counseling, and treatment planning to children and their care givers.
- In Person, in the home, community-based, in office and telehealth to meet the needs of our clients.
- Complete an assessment of needs and recommendations for further medically necessary services, such as rehabilitation services. Services are delivered in a trauma informed, culturally and linguistically competent manner.
- Work with family members, advocates and other salient parties to identify goals of treatment. Provide observations and information to collateral contacts for the purpose of the progress and tracking of the Treatment Plan goals.
- Complete all required documentation for submission to the Supervisor within the required timeframes.
Requirements and Education
- Masters degree in Social Work, Counseling, Psychology or related field.
- 1 year of related experience.

- NYS drivers license with a reliable vehicle, as work requires up to 50% community travel/client home visits.

Description
As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need

As a Greenhouse Coordinator, you will development and implementation of the botanical classroom instructional plans for students. You will report to the Transition Learning Coordinator and you will work onsite.

- Follow school and agency procedures and policies for reporting incidents. This includes behavior incident reports (IR) and incidents such as child/sexual abuse, substance abuse, harassment, bullying, and violence.
- Establish relationships with parents and family members to create a partnership around student treatment, student behavior, and academic progress.
- Participate in classroom grade level, and behavior/treatment meetings as well as professional training and development sessions (e.g. TCI, CPR, emergency procedures and other agency requirements.
- Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
- Facilitate partnership between the greenhouse activities and gardens through home and school education programs by coordinating community workshops, recruiting home garden volunteers.
- Work with other teachers, clinicians, administrators, medical staff and other school staff to provide integrated support, assistance, and treatment to students.

As a Skillbuilder, you will provide targeted skill training to help with independent living and social skills. As a Residential Program Manager, you will manage and oversee child care and milieu activities across the residential system, promoting a therapeutic environment that supports active permanency planning and work. You will report to the Residential Executive Program Director and will work onsite.

- Monitor serious incidents within the programs and respond to our licensing agency with corrective action.
