Description

As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). You will report to the Director of the Behavioral Health Center. Sessions may be conducted in the office or via Tele-mental Health as clinically indicated.

Responsibilities:

• Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
• Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
• Collaborate with on-site prescriber, other medical and mental health providers, schools and any other community supports for client care.
• Attend Behavioral Health Center staff meeting twice a month.
• Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
• Provide referrals and linkages to community supports, resources, and services.

Requirements and Education:

• Master's degree in Social Work or Mental Health Counselor.
• Experience in human services, case management, line staff at a residential program, or any other related direct care within human services, both pre and post master degree, will be considered.
• License and/or Limited Permit eligible, have a limited permit in discipline or Fully Licensed (LMHC/LCSW/LCSW-R), preferred.

• Supports the development and customization of resumes, cover letters, and thank you letters.
• Work with participants to develop and hone interviewing skills (including disclosing disabilities and discussing accommodations if applicable) with employers. Develop a placement or job-searching plan in accordance with the participant’s vocational interests and abilities.
• Assess and assists participants with any needed job-seeking skills including, but not limited to, locating employment opportunities and supporting them to complete job applications. Are you looking to make a difference in the lives of youth and families struggling with behavioral health issues? Do you want to develop and supervise your own team and mentor your staff to improve their clinical skills? As the Team Lead of Home Based Crisis Intervention (HBCI), you will provide clinical supervision, support, and mentorship to your team to help them change lives. The HBCI team provides short-term (up to 6 weeks), intensive intervention to youth and families who are in crisis. We work with parents and caregivers to help them help their children. The short-term format means the work is fast-paced and outcome oriented, which means you and your team will produce meaningful outcomes in a short period of time. You will report to the Chief of Crisis Services and work on-site with travel required.

Responsibilities:

• Provide clinical supervision to the team and be responsible for allocation of staff time; provide training and ensure all team members are kept informed of issues.
• Monitor caseload size and coordinate program intakes and discharges.
• Create rotating call schedule for after-hours / weekend coverage by team members.
• Provide education and outreach to the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.
• Develop collaborative relationships with other agencies, schools, hospitals, primary care providers, and local government agencies to help meet the needs of this population so they can achieve and maintain their independence.
• Assist primary clinicians to conduct needs assessments, safety planning and wellness management planning with participants. Connect participants and families with the services and support they need. Provide psychoeducation regarding mental health diagnosis and treatment. Develop treatment using a shared decision-making process with clients.
• Assist primary clinicians to respond to on-call needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling cognizant of family needs, with 24/7 availability nights, weekends and holidays.
• Promote safety of the child, family, and community in the delivery of crisis intervention services.
• Assist primary clinicians to conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment while developing a positive working relationship with the families.
• Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves and prepare them for transition and service closure.

Requirements and Education:

• Requires a Master’s degree and at least one year of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.
• Must be a licensed mental health professional. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist.

Credible Messenger Counselor

As a Credible Messenger Counselor, you will develop transformative mentoring relationships with youth in the justice system. Credible Messenger Mentoring for youth in the juvenile justice system is a transformative process through which individuals from similar backgrounds, especially men and women who have personal experience in the justice system, engage youth in structured and intentional relationships that help them change their attitudes, belief, and actions. You will also provide therapeutic and case management services for the youth and the family. You will report to the Assistant Director of Juvenile Justice Programming in this hybrid position.

Responsibilities:

• Maintain a caseload of nine youth and and meet with each youth weekly in their home or a mutually agreed upon community setting, as well as providing outreach phone calls and support to caregivers.
• Advocate and support youth as they interact with justice systems, education, employment, health, and social service systems.
• Participate in Credible Messenger training and consultation opportunities to develop restorative justice and mentoring skills, as well as learning about promoting a lifestyle of sustained transformation.
• Advocate and guide youth as they interact with justice systems, education, employment, health, and social service systems.
• Establish and maintain working relationships with colleagues, Probation Department, service providers, and community resources.
• Maintain case record and complete all paperwork: work-based Learning documentation, status sheets, progress notes, service plans in accordance with program contract, agency policy, COA and state mandates.
• Coordinate and implement work-based Learning interventions with each youth, including the issuing of stipends to the youth and obtaining all necessary signatures to verify accuracy of implementation.

• A Bachelor's degree in Social Work or related field with at least three to five years of experience.
• At least one to two years of demonstrated successful “lived experience” within the Justice or Child Welfare Systems.
• A valid Drivers License, and access to a reliable vehicle to conduct home visits and outreach to youth, as well as transporting youth.

POSITION OVERVIEW

As a Skillbuilder, you will provide targeted skill training to help with independent living and social skills. Goals that the Skill Builder will work on could be Career Planning, Self-Care, Daily Living, Housing, and Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures, and using their coping skills when feeling dysregulated. You will report to the Program Coordinator and you will work onsite.

Responsibilities:

• At a minimum, 50% of the work week is spent face-to-face with clients to provide services designated in the Treatment Plan.
• Provide services based on the youth's needs and preferences as identified in the Treatment Plan.
• Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
• Complete all required documentation and submit it to the Supervisor within the required timeframes.
• Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with the case manager to find resources in the community for educational and vocational skills and programming for discharge.

Requirements and Education:

• High School Diploma/GED required
• At least 2 years of experience working with children
• Valid driver's license required
