Respite Workers Needed in Albany for Youth in care.

Per Diem (As Needed) Shift

As a Respite Worker for the Permanency Resource Center, you will provide respite services to youth in adoptive, guardianship and kinship families. Respite may also be available to families as part of the service plan and can occur at parent support groups or other PRC events and will be designed to be consistent with the children's needs and interests. Outings or activities with individual youth will be coordinated with the supervisor and parent. Respite provided as part of a group activity at a support group will be designed to allow parents the opportunity to have time separate from their children while they attend a support group. You will report to the Case Manager Supervisor or designated supervisor at the event. You will work onsite with youth in the community.

Responsibilities:

• Provide supervision for youth while parents attend support group.
• Provide respite as assigned to a youth as part of their service plan which can include activities in the community or in the youth’s home if that is the agreed upon plan.
• Participate in family engagement activities to provide added support to youth.
• Co-lead support groups for youth.
• Maintain records including required data collection information according to the program standards and expectations.

Requirements and Education:

• High school diploma or GED with 3 years related experience
• Must be able to communicate effectively and develop positive working relationships
• A valid NYS driver’s license

POSITION SUMMARY

This position is responsible for working closely with youth, families, and all team members in developing behavior plans and individualized crisis management plans. This position will provide these services in the RTF, in family homes, and in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Employees are expected to be awake and alert while working.

Participate in every family planning meeting and every Child and Family Team meeting for each family during RTF admission.

Create preventive, behavioral, and crisis plans for each youth in the RTF. These plans include self-care plans, home crisis plans, coping plans, and ICMPs. Review and/or revise individual safety plans after every significant incident and at every treatment review.

Maintain the RTFs analysis of incident trend data, such as frequency/duration; individuals involved (residents and staff); time, location, and precipitating factors, to refine quality improvement efforts. Report monthly at team meeting.

Expand/analyze all incident data trends to feed back for behavior plans.

Ensure all staff members who potentially may be involved in requesting and/or participating in a restraint have completed an OMH approved crisis management training curriculum.

Offer ongoing mentoring and coaching to direct care staff in area of trauma informed care and de-escalation techniques.

Use ARC clinical knowledge to promote recovery oriented, strength-based approach with an emphasis on asset skill and resiliency building in daily therapeutic activities.

Debrief with each young person post restraint. Participate in post restraint staff debrief meetings.

Maintain contact with families and provide psycho-educational and skill building activities for families so that a strength based behavioral support approach is maintained when the child returns to a community setting. Spend time with the family in the family home or in the community to practice learned skills in real time.

Collaborate with school staff on individualized behavior plans. Attend school meetings as assigned.

Ensure approved restraint techniques are used when restraint is necessary. Observe all restraints if present and feasible.

Create behavior plans with consultation of the team psychologist, youth and family. Communicate and revise behavior plans with the treatment team.

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

Description

Are you looking to make a difference in the lives of youth and families struggling with behavioral health issues? Do you want to develop and supervise your own team and mentor your staff to improve their clinical skills? As the Team Lead of Home Based Crisis Intervention (HBCI), you will provide clinical supervision, support, and mentorship to your team to help them change lives. The HBCI team provides short-term (up to 6 weeks), intensive intervention to youth and families who are in crisis. We work with parents and caregivers to help them help their children. The short-term format means the work is fast-paced and outcome oriented, which means you and your team will produce meaningful outcomes in a short period of time. You will report to the Chief of Crisis Services and work on-site with travel required.

Responsibilities:

• Provide clinical supervision to the team and be responsible for allocation of staff time; provide training and ensure all team members are kept informed of issues.
• Monitor caseload size and coordinate program intakes and discharges.
• Create rotating call schedule for after-hours / weekend coverage by team members.
• Provide education and outreach to the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.
• Develop collaborative relationships with other agencies, schools, hospitals, primary care providers, and local government agencies to help meet the needs of this population so they can achieve and maintain their independence.
• Assist primary clinicians to conduct needs assessments, safety planning and wellness management planning with participants. Connect participants and families with the services and support they need. Provide psychoeducation regarding mental health diagnosis and treatment. Develop treatment using a shared decision-making process with clients.
• Assist primary clinicians to respond to on-call needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling cognizant of family needs, with 24/7 availability nights, weekends and holidays.
• Promote safety of the child, family, and community in the delivery of crisis intervention services.
• Assist primary clinicians to conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment while developing a positive working relationship with the families.
• Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves and prepare them for transition and service closure.

Requirements and Education:

• Requires a Master’s degree and at least one year of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.
• Must be a licensed mental health professional. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist.
• Must have a valid driver’s license.

As a Youth Peer Advocate, you will be an ally, mentor, and advocate for clients and families as they contend with the difficulties and uncertainties of mental illness. You will use lived experience to contribute to treatment planning for each participant, and facilitating the team’s development and maintenance of a culture of respect. The Youth Assertive Community Treatment team (ACT) Program is an innovative treatment program. The program serves children with mental illness or serious emotional disturbance and their families. You will report to the Director of Youth Act and work onsite with community-based travel required.

Responsibilities:

• Engage individuals and establish trusting, collaborative relationships directed toward the goals clients identify on their treatment plans
• Contribute to client treatment plans, advocating for clients' needs
• Participate in daily treatment team meetings and work directly with other team members to provide support and assistance to program participants.
• Provide individual face-to-face support to youth and families receiving services
• Spend 70% or more of scheduled work hours in community settings
• Meet with family, youth, and collateral contacts to provide psychoeducation about serious mental illness
• Be a role model, educate recipients about self-help techniques and self-help group processes, teach effective coping strategies based on personal experience, teach symptom management skills, assist in clarifying rehabilitation and recovery goals, and assist in the development of community support systems and networks.

Requirements and Education:

• High School Diploma or GED, Associates Degree preferred
• A valid New York State driver’s license.
• Complete and obtain the NYCPS-Provisional certification for review by the New York Peer Specialist Certification Board (NYPSCB) within the agreed-upon time of start date.
