Per Diem Clinician/Social Worker - Mobile Crisis

Location: Schenectady, NY or Queensbury, NY

Day, Evening, and Weekend Shifts available

As a Clinician for Mobile Crisis, you will provide crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. You will work on-site with travel to the community and individuals' homes.

Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.
• Provide short-term crisis intervention and stabilization services to adults, children, adolescents, and their families in the home or community (wherever the individual is experiencing a crisis).

Requirements and Education:

• Looking for applicants with their LMSW, LMHC, LCSW, LCSW-R licensure
• Requires a Masters' degree (MSW/MHC) and two years of related experience
• Must have a valid NYS Drivers License and be willing and able to drive to clients' homes and communities under a variety of circumstances and weather conditions. Must be willing to work in many settings, including clients' homes and other community locations where there may be pets, cigarette smoke, etc.

Director, Assertive Community Treatment (ACT) Adult

As a Director of ACT, you will have the opportunity to significantly improve the lives of adults who have a severe and persistent mental illness that seriously impairs their functioning in the community. You will oversee a multi-disciplinary team that provides services and support to adults living in the community Your duties include the management of the program, staff matters, client referrals, program manual, and program systems. You will promote the agency's commitment to excellence in the provision of mental health services that are strength-based, recovery-oriented, shared decision-making process, along with culturally competent and sensitive. ACT is a program that provides services through less traditional service delivery methods. This is very much a "meet the client in their community" approach. You will report to the Chief Officer of Crisis Response Service and work onsite.

Responsibilities:

• Provide overall supervision to the team and is responsible for allocation of staff time
• Ensure all team members are kept informed of issues involving participants and their families
• Monitor caseload size and coordinates program intakes and discharges
• Facilitate, organize, and document daily team meetings
• Provide training and supervision for the Staff
• Make rotating on-call schedule for after hours / weekend coverage by team members
• Provide after hour and weekend telephonic backup for on-call staff
• Educate the community about Assertive Community Treatment (ACT); philosophy, program outcomes, theoretical background
• Develop and maintain collaborative relationships with other agencies, schools, colleges, institutions, hospitals, primary care providers, housing authorities, and law enforcement agencies to help meet the needs of this population so they can achieve and maintain their independence
• Ensure staff are adhering to the fidelity of the program
• Participate in all required trainings as directed by funding source and ensure that staff are also in compliance with all funding source training
• Be a primary clinician for a subset of program participants. In this role of Primary Clinician, the Team Leader will:
  • Be a point person for the participant and their family
  • Conduct needs assessments
  • Engage in safety planning and wellness management planning with participants
  • Connect participants and families with the services and supports they need
  • Provide psychoeducation regarding mental health diagnosis and treatment
  • Develop and modify treatment using a shared decision-making process with clients

Requirements and Education:

• A Master’s degree and license in a helping profession- Social Work, Mental Health Counseling, Marriage and Family Therapy, Creative Arts Therapy, etc.OR licensure/registration as a RN (RNs are encouraged to be master’s level or nurse practitioner), or physician.
• At least two years of progressively responsible related experience.

As a Clinician in the Intensive Aftercare Prevention program, you will provide home-based clinical services to children and their families to help preserve or reunite them following Northeast Parent & Child Society's stated purpose and mission of protecting children, preserving families, and strengthen communities. The Intensive Aftercare Prevention Program (IAPP) is a home and community-based program assisting families who have children in foster care or children who are at risk of being placed in foster care. Responsibilities:

• Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed)
• Complete necessary all records and client information following agency, county, and state policy and funding requirements

As an English /Language Arts Teacher, you will develop and implement the English/Language Arts classroom instructional plans for students. You will report to the principle and will work onsite.

Responsibilities:

• Provide subject area instruction (English/Language Arts) that follow the NYS K-12 English/Language Arts curricula and prepares students for exit examinations and statewide assessments.
• Develop and implement English/Language Arts instructional materials to provide individual and small group instruction to students of various ability levels. At the high school level develop lessons, teach, and prepare students for examinations in English/Language Arts.
• Develop and implement a classroom instructional plan with the Special Education Teacher that integrates the students IEP goals and objectives, learning style. as well as accommodations and modifications that are needed.
• Develop and implement with the Special Education Teacher, individual and classroom behavior plans for managing student behavior to maintain discipline in the Classroom; assist classroom staff with crisis prevention and intervention techniques; assist classroom with de-escalation techniques; teach students appropriate behavior strategies.
• Use the classroom PBIS universal plan and Tier 2 and 3 strategies for selected students.
• Manage and coordinate the safety and well-being of students through the use of established classroom and schoolwide processes and procedures; supervise classroom staff implementing these with their work with students throughout the building.
• Follow school and agency procedures and policies for reporting incidents. This includes behavior incident reports as well as incidents such as child/sexual abuse, substance abuse, harassment, bullying, and violence.
• Establish positive relationships with parents and family members to create a partnership around student treatment, student behavior, and academic progress.
• Participate in classroom grade level, and behavioral/treatment meetings as well as professional training and development sessions(e.g. TCI, CPR, emergency procedures and other agency requirements).
• Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
• Provide leadership to assigned teacher assistants in various clerical and non-instructional duties as need for your classroom.
• Work collaboratively with other teachers, clinicians, administrators, medical staff, and other school staff to provide integrated support, assistance, and treatment to students.

Requirements and Education:

• A Master's degree in Special Education or subject area and the ability to work with students who demonstrate serious emotional and behavioral challenges, ages 6-21. a Bachelor's degree with enrollment in a program that leads to a master's degree may be considered.
• A NYS certification in Special Education or subject area, or enrollment in a program that leads to a NYS teacher certification.

• Requires a Masters' degree (MSW/MHC) and two years of related experience
• Willing to pass and obtain licensure within 6 months of hire - Required
• Must have a valid NYS Drivers License and be willing/able to drive to clients' homes and communities under a variety of circumstances and weather conditions. Must be willing to work in many settings, including clients' homes and other community locations where there may be pets, cigarette smoke, etc.

POSITION OVERVIEW

Clinical Case Manager, Intensive Aftercare Prevention Program

As a Clinical Case Manager in the Intensive Aftercare Prevention Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.

Responsibilities:

• Provide home and community services to individuals, groups, and families as prescribed in the treatment plan for those enrolled in our Family Preservation Services.
• Educate families in a variety of social-emotional and parenting skills such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc.
• Work in partnership with a Master’s level clinician to provide needed services to an assigned caseload of families.
• Complete and maintain appropriate case records through the completion of intake, assessments, progress notes, uniform case records, reports, etc. following agency, county, and state policy and funding requirements.
• Participate in the comprehensive assessment and creation of the treatment plan for assigned cases to determine and address service needs and ensure the provision of regular treatment plan meetings and service plan reviews.
• Provide liaison contact with collateral resources such as referring agencies, schools, family courts, and other agency personnel serving on behalf of a child and family as it relates to their treatment.
• You will work Monday through Friday from 8:30 am to 5:00 pm with a flexible schedule that includes evenings and a rotating on-call schedule.

Requirements and Education:

• Bachelor's degree in social work, Sociology, Criminal Justice, Psychology, Human Services, or a related field;
• Experience working with at-risk youth and families;
• Home and community-based experience is a plus.
