As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.

As a Skillbuilder, you will provide targeted skill training to help with independent living and social skills. Goals that the Skill Builder will work on could be Career Planning, Self-Care, Daily Living, Housing, and Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures, and using their coping skills when feeling dysregulated. You will report to the Program Coordinator and you will work onsite.

Responsibilities:

• At a minimum, 50% of the work week is spent face-to-face with clients to provide services designated in the Treatment Plan.
• Provide services based on the youth's needs and preferences as identified in the Treatment Plan.
• Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
• Complete all required documentation and submit it to the Supervisor within the required timeframes.
• Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with the case manager to find resources in the community for educational and vocational skills and programming for discharge.

As a Peer Advocate, you will in the Permanency Resource Center to assist adoptive, guardianship and kinship/custodial families and case managers in helping families and youth access services. You will assist in the development, scheduling and provision of family engagement activities and support groups. You will report to the Permanency Resource Center Case Manager Supervisor and will work both onsite and remote with community-based travel required.

Responsibilities:

• Provide families with information about resources in the community for services, ongoing support and linkages.
• Assist parents and caregivers in identifying any barriers to accessing community resources.
• Assist parents and caregivers with documentation and other steps needed to access resources and support from either public or private entities.
• Provide strategies and intervention suggestions to parents/caregivers whose children's challenges create safety concerns.
• Provide support to the permanency care managers.
• Collaborate with school personneland attend meetings when indicated to support the permanency care managers.
• Help with transportation needs.
• Coordinate Family Engagement Events.
• Schedule child care staff for support groups.

As a Peer Support Specialist, you will be a member of the Mobile Team, assisting the clinicians on Mobile visits and providing support and advocacy to individuals in crisis. You will also be a resource to the clinicians and case manager, through sharing their knowledge of common challenges individuals with SPMI endure and sharing knowledge about community resources. You will be an individual (adult) with lived experience with mental illness. You will report to the Clinical Site Supervisor and you will work on-site.

Responsibilities:

• Collaborate with service providers involved with individuals with SPMI.
• Provide information and peer to peer support to individuals who are served by the program.
• Educate and empower clients and their supports regarding their roles in developing crisis plans and in crisis prevention.
• Educate service providers regarding an individual's role in developing a crisis plan for themselves and in crisis prevention. Ensure that the individual is an integral part of the crisis planning process.
• Inform service providers (including Mobile Crisis and PCFC providers) regarding person- centered and strength-based practices. Maintain and model a focus on strengths.
• Utilize clinicians and clinical supervisor for advocacy for individuals served.
• Ensure that clients are satisfied with the services they are receiving and, if not, help them advocate for what they need.
• Educate individuals and primary caretakers on how to advocate if services are not meeting their needs.
• Be the liaison to other local supports in the community.
• Link individuals to Peer Support Services activities such as support groups, social events, newsletter.

Credentialed Family Peer – Schenectady, NY – Full Time

We are looking for a credentialed Family Peer to join our Schenectady Family Support program. The Family Peer provides an array of formal and informal services to families raising children who are experiencing social, emotional, developmental, substance abuse and/or behavioral challenges in their home, school, placement, or community.

Family Peers must have lived experience as a parent or primary caregiver of a child with serious emotional disturbance, and have experience advocating for services in at least two service systems (mental health, education social services, juvenile justice).

Family Peer will provide individual and group support services to the parent or caregiver for the benefit of the child. Services include the following.

• Engagement, Bridging, and Transition Support

• Self-Advocacy, Self-Efficient, and Empowerment

• Parent Skill Development

• Community Connections and Natural Support

• Promoting Effective Family-Driven Practice

ESSENTIAL DUTIES AND RESPONSIBILITIESinclude the following. Other duties may be assigned.

• Identify goals with the parent/guardian through the use of the Family Assessment of Needs and Strengths.

• Maintain case notes that outline activities to accomplish identified goal(s), and identify the amount of time each week that will be designated to work on goal(s).

• Conduct regular family support groups on various topics and/or resources for families.

• Provide information and parent-to-parent support to parents or caretakers

• Educate family members on the family’s role in developing service plans, and educate/empower them on how to advocate if services are not meeting their needs.

• Participate in trainings that support the role (i.e., PEP, etc.).

EDUCATION and/or EXPERIENCE

• High School Diploma or GED required.

• Must be a parent or caregiver of child with serious emotional disturbance, and have experience advocating for services in at least two service systems (Mental Health, education social services, juvenile justice).

• Must have reliable transportation and a valid NYS drivers license.

• Strong computer and writing skills essential.

CERTIFICATION / LICENSURE

• Drivers license is required.
• Credentialing as a Family Peer through Families Together, NY, is required. Agency will support this process upon hire if candidate is not currently credentialed.

As a Youth Peer Advocate, you will be an ally, mentor, and advocate for clients and families as they contend with the difficulties and uncertainties of mental illness. You will use lived experience to contribute to treatment planning for each participant, and facilitating the team’s development and maintenance of a culture of respect. The Youth Assertive Community Treatment team (ACT) Program is an innovative treatment program. The program serves children with mental illness or serious emotional disturbance and their families. You will report to the Director of Youth Act and work onsite with community-based travel required.

Responsibilities:

• Engage individuals and establish trusting, collaborative relationships directed toward the goals clients identify on their treatment plans
• Contribute to client treatment plans, advocating for clients' needs
• Participate in daily treatment team meetings and work directly with other team members to provide support and assistance to program participants.
• Provide individual face-to-face support to youth and families receiving services
• Spend 70% or more of scheduled work hours in community settings
• Meet with family, youth, and collateral contacts to provide psychoeducation about serious mental illness
• Be a role model, educate recipients about self-help techniques and self-help group processes, teach effective coping strategies based on personal experience, teach symptom management skills, assist in clarifying rehabilitation and recovery goals, and assist in the development of community support systems and networks.

Requirements and Education:

• High School Diploma or GED, Associates Degree preferred
• A valid New York State driver’s license.
• Complete and obtain the NYCPS-Provisional certification for review by the New York Peer Specialist Certification Board (NYPSCB) within the agreed-upon time of start date.

• Facilitate the Peer Run Social Club.
• Provide coverage for Saratoga County Warm line as needed.
• Provide training and teach skills needed to navigate mental health services in Saratoga County to identified clients.
• Promotes socialization, recovery, self-sufficiency, development of natural supports as well as coping skills and goal setting for those individuals with mental health issues.
• Act as an advocate by assisting individuals with housing issues, medical concerns, therapy or medication related problems, employment goals, and basic budgeting skills.
• Assist with benefits counseling.
• Engage participants in shared decision making around services and recovery planning.
• Function as part of a participant’s treatment to ensure integrated, collaborative care.
• Assist participants with the identification of activities that promote a sense of prosocial community engagement.
• Participate in regularly scheduled supervisions.
• Complete daily case notes.
• Complete all required agency trainings as well as trainings and certifications set forth by the New York State Office of Mental Health.
• Must possess strong interpersonal boundaries.

• High School or GED required.
• At least two years’ experience with the Mental Health system in Saratoga County.
• Per regulatory requirements, must have life experience with mental health and/or recovery from
• substance abuse.

• Peer Empowerment Credential required for Adult HCBS.
• Must have a valid NYS Driver’s license. Ability to operate agency vehicles is required.
