As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

• Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
• Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
  • Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
  • Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
  • Work w/children, youth, families, and service providers to create a comprehensive Plan of Care
  • Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
  • Convene and conduct Interdisciplinary team meetings to review Plan of Care; identifying progress, continued needs, and confirming ongoing eligibility
  • Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
  • Use NY State and Health Home information systems to input required Health Home documentation
  • Comply with Medicaid and program billing regulations
  • Complete all required and recommended trainings

Requirements and Education:

• Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
• Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
• You have a valid driver's license and automobile that is insured following New York State Requirements
• You will work with individuals of different backgrounds, age, ethnicities, life positions, and socio-economic statuses.

POSITION OVERVIEW

• Provide services to a caseload of children and families.
• Provide family support, transportation, and coordination of services to client families.
• Maintain cultural sensitivity, competence, and client confidentiality at all times.

• Maintain good working relationships with DSS/Support Unit caseworkers.
• Complete progress notes or other case record notes in a timely manner.
• Provide opportunities for new employees to shadow appointments.
• Provide basic instruction on program philosophy and information on community resources to new employees.
• Provide new employee orientation training.
• Interface with SCDSS personnel to receive referrals and ensure service provision including scheduling, tracking and overseeing completion of progress notes.
• Supervise the Service Support Worker
• Carry a modified caseload
• Serve as a back up to the Service Support Worker when needed.
• Provide non-clinical guidance to FSWs in the absence of the coordinator.
• Assist in the hiring of program staff as requested.

As a Science Teacher, you will develop and implement Science classroom instructional plans for students. You will provide subject area instruction (Science) that follow the NYS K-12 Science Curricula and prepare students for exit examinations and statewide assessments. You will report to the Assistant principal and you will work onsite.

Responsibilities:

• Develop and implement science instructional materials to provide individual and small group instruction to students of various ability levels, and at the high school level develop, teach, and assess laboratory experiences for Earth Science, Living Environment, and/or Environmental Science.
• Develop and implement classroom instructional plans with the Special Education Teachers that integrates the student's IEP goals and objectives, learning styles, and accommodations and modifications that are needed.
• Work with the Special Education Teachers on individual and classroom behavior plans for managing student behavior to maintain discipline in the classroom.
• Use the classroom PBIS universal plan and Tier 2 and 3 strategies for selected students.
• Coordinate the safety and well-being of students through the use of established classroom and schoolwide processes and procedures; supervise classroom staff in implementing these with their work with students throughout the building.
• Follow school and agency procedures and policies for reporting incidents. This includes behavior incident reports (IR) and incidents such as child/sexual abuse, substance abuse, harassment, bullying, and violence.
• Establish relationships with parents and family members to create a partnership around student treatment, student behavior, and academic progress.
• Participate in classroom grade level, and behavior/treatment meetings and professional training and development sessions (e.g. TCI, CPR, emergency procedures and other agency requirements).

Per Diem Clinician, Permanency Resource Center (PRC)

As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicated to individuals, families and groups in a Permanency Resource Center. This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases. Therapy is to be provided in the community or in office, and is paid for by the program, not through insurance reimbursement. You will report to the Licensed Clinician and you will work in a hybrid model with community-based travel required.

Responsibilities:

• Provision of outpatient therapy remotely or in person with youth or caregivers within the Permanency Resource Center system.
• Document the contact within the Evolv system.
• Attend and participate in individual supervision on at least a monthly basis and more frequently if the acuity of the assigned cases requires this.
• Provide therapeutic services to individuals assigned to case load.
• Provide case management services as clinically indicated.
• Provide referral, collateral and linkage services.
• Record and maintain case records according to program standards.

Requirements and Education:

• A Master’s degree in one of the healing arts.
• A minimum of a limited permit in social work, family therapy, mental health counseling, or the healing arts.
• A valid NYS driver's license.
