Position Summary

Provide daily therapeutic care and programming according to Agency and other governing bodies (LDSS, OCFS, CSE, etc.) standards and requirements for adolescent clients who are emotionally disturbed.

Essential Duties and Responsibilities

• Maintain the physical order of the building and therapeutic environment of the units.
• Develop and maintain schedules within the assigned campus residential unit, ensuring that there is adequate staff to meet regulatory requirements and safety requirements.
• Responsible to ensure that youth in our care are being supervised according to regulatory and programmatic standards.
• Design, implement and supervise program activity which will lead to the attainment of goals described within treatment plans.
• Supervise and evaluate the work of residential staff assigned to residential programs.
• Maintain records as directed by the Residential Program Manager.
• Participate in the interviewing and selection of new staff members.
• Participate in the training of residential staff.
• Conduct residential staff meetings.
• Participate in the evaluation of youth referred to the Agency for residential care.
• Conduct the financial activity of the residential unit in accordance with guidelines established by the Residential Program Manager and the Director.
• Responsible for managing program funds, purchasing food, clothing, household supplies, managing youth allowances
• Participate/organize program and agency wide special events and services.
• Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food (cooking when necessary), medical care, a reliable and nurturing environment.
• Participate in program on call rotation

Education and Experience
Requires minimum of HS diploma or equivalent and at least three to four years of relevant experience.

As a Case Manager, you will work with the children in care, the Department of Social Services, foster families, and birth families in the journey toward achieving permanency in multiple settings. You will report to the Program Coordinator and work on-site with some travel.

Responsibilities:

• Work with foster and birth families to coordinate all appointments for children, including providing transportation if another plan cannot be found.
• Attend all client-related meetings, including Counsel on Special Education, Termination of Parental Rights, staff meetings, service provider meetings, emergency meetings, court, and others.

As a Transition Coordinator, you will coordinate transition services and instruction to enable students to reach their fullest potential and be able to live, learn and earn in their community as an adult. You will report to the Program Manager and will work onsite.

Responsibilities:

• Work with NHS staff, Clinical staff, Parents/guardians, outside service providers and identified students to:
  • Design a realistic discharge plan that is guided by each youth’s wishes, hopes and dreams
  • Coordinate your client driven discharge planning process with local OCFS, OMH, and NYSED representatives that supports a client-driven plan which integrates housing, health and medical care, education and employment needs.
  • Coordinate a set of vocational and transitional related activities including instruction, community experiences, employment and volunteer experiences, and the acquisition of daily living skills.
  • Examine/identify post secondary vocational training options
  • Respectfully honor each youth’s feelings about past and current relationships and ensure that when needed individuals will have the opportunity to work on issues of grief, loss and enduring relationships
  • Complete a Permanency Pact and identify specific people who agree to offer life long supports (either related or not)
  • Develop a Circle of Supportand identify specific people who agree to advocate for and support the student during the transition process
• Promote active involvement of identified students in planning and implementing the transition component of their IEP.
• Coordinate services to ensure that each identified student has an accurate Work Skills Employability Profile and earns a Regents Certificate of Work Readiness upon graduation from the Neil Hellman School
• Coordinate transition portfolios for each identified student.
• Provide 6 months of follow up and maintain post-NHS discharge statistics for each identified student.

Are you looking to make a difference in the lives of youth and families struggling with behavioral health issues? Do you want to develop and supervise your own team and mentor your staff to improve their clinical skills? As the Team Lead of Home Based Crisis Intervention (HBCI), you will provide clinical supervision, support, and mentorship to your team to help them change lives. The HBCI team provides short-term (up to 6 weeks), intensive intervention to youth and families who are in crisis. We work with parents and caregivers to help them help their children. The short-term format means the work is fast-paced and outcome oriented, which means you and your team will produce meaningful outcomes in a short period of time. You will report to the Chief of Crisis Services and work on-site with travel required.

Responsibilities:

• Provide clinical supervision to the team and be responsible for allocation of staff time; provide training and ensure all team members are kept informed of issues.
• Monitor caseload size and coordinate program intakes and discharges.
• Create rotating call schedule for after-hours / weekend coverage by team members.
• Provide education and outreach to the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.

• Assist primary clinicians to conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment while developing a positive working relationship with the families.
• Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves and prepare them for transition and service closure.

Requirements and Education:

• Requires a Master’s degree and at least one year of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.

The Behavioral Health program and the Youth Assertive Community Treatment (ACT) program are seeking a Psychiatric Nurse Practitioner for a shared role working between our Schenectady NY location and our Rensselaer NY location. This position is responsible for providing comprehensive Psychiatric services to adults and youth who are enrolled in our outpatient clinic program and a 28-person caseload of youth enrolled in our Youth ACT program. This position is full-time, 40 hours per week. Must be able to seechildren.

Responsibilities:

• Provide comprehensive psychiatric services to youth enrolled in Youth ACT and adults and youth enrolled in our BehavioralHealth Clinic as assigned
• Provide consultation to staff on diagnostic and treatment questions
• Participate in formal and informal training and education to staff
• Prescribe medications, monitor, and review of drug regimen for enrolled individuals
• Participate in quality assurance activities, consult with the Youth ACT Team in developing and reviewing service plans for all Youth ACT enrolled individuals
• Provide clinical expertise, consultation, and technical assistance to the Youth ACT Team/Behavioral Health, cooperate with the County and participate in any quality audit and utilization review

• Must be fully licensed Nurse Practitionerin the State of New York.

As a Program Supervisor, you will be responsible for the administrative and programmatic oversight of a Foster Care team. You will oversee staff, the foster families associated with the office and maintain agency resource relationships within the counties the foster care program supports. • Supervise multistage foster care treatment teams in areas of intake and ongoing treatment planning for foster youth and foster families.

As a Science Teacher, you will develop and implement Science classroom instructional plans for students. As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

• Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
• Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
  • Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
  • Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
  • Work with children, youth, families, and service providers to create a comprehensive Plan of Care
  • Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
  • Convene and conduct Interdisciplinary team meetings to review the Plan of Care; identify progress, and continued needs, and confirm ongoing eligibility
  • Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
  • Use NY State and Health Home information systems to input required Health Home documentation
  • Comply with Medicaid and program billing regulations
  • Complete all required and recommended training

As a Credible Messenger Counselor, you will develop transformative mentoring relationships with youth in the justice system. Credible Messenger Mentoring for youth in the juvenile justice system is a transformative process through which individuals from similar backgrounds, especially men and women who have personal experience in the justice system, engage youth in structured and intentional relationships that help them change their attitudes, belief, and actions. As a Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. Per Diem Clinician, Permanency Resource Center (PRC)

As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicated to individuals, families and groups in a Permanency Resource Center. This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases. Therapy is to be provided in the community or in office, and is paid for by the program, not through insurance reimbursement. You will report to the Licensed Clinician and you will work in a hybrid model with community-based travel required.

Responsibilities:

• Provision of outpatient therapy remotely or in person with youth or caregivers within the Permanency Resource Center system.
• Document the contact within the Evolv system.
• Attend and participate in individual supervision on at least a monthly basis and more frequently if the acuity of the assigned cases requires this.
• Provide therapeutic services to individuals assigned to case load.

Respite Workers Needed in Albany for Youth in care.

Per Diem (As Needed) Shift

As a Respite Worker for the Permanency Resource Center, you will provide respite services to youth in adoptive, guardianship and kinship families. Respite may also be available to families as part of the service plan and can occur at parent support groups or other PRC events and will be designed to be consistent with the children's needs and interests. Outings or activities with individual youth will be coordinated with the supervisor and parent. Respite provided as part of a group activity at a support group will be designed to allow parents the opportunity to have time separate from their children while they attend a support group. You will report to the Case Manager Supervisor or designated supervisor at the event. You will work onsite with youth in the community.

Responsibilities:

• Provide supervision for youth while parents attend support group.
• Provide respite as assigned to a youth as part of their service plan which can include activities in the community or in the youth’s home if that is the agreed upon plan.
• Participate in family engagement activities to provide added support to youth.
• Co-lead support groups for youth.
• Maintain records including required data collection information according to the program standards and expectations.

Clinical Case Manager, Intensive Aftercare Prevention Program

As a Clinical Case Manager in the Intensive Aftercare Prevention Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and you will work with a partner (clinician) in a combination of on-site and remote with travel to client's homes required.

Responsibilities:

• Provide home and community services to individuals, groups, and families as prescribed in the treatment plan for those enrolled in our Family Preservation Services.
• Educate families in a variety of social-emotional and parenting skills such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc.
• Work in partnership with a Master’s level clinician to provide needed services to an assigned caseload of families.
• Complete and maintain appropriate case records through the completion of intake, assessments, progress notes, uniform case records, reports, etc. following agency, county, and state policy and funding requirements.
• Participate in the comprehensive assessment and creation of the treatment plan for assigned cases to determine and address service needs and ensure the provision of regular treatment plan meetings and service plan reviews.
• Provide liaison contact with collateral resources such as referring agencies, schools, family courts, and other agency personnel serving on behalf of a child and family as it relates to their treatment.
• You will work Monday through Friday from 8:30 am to 5:00 pm with a flexible schedule that includes evenings and a rotating on-call schedule.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

• Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.

As a Clinician, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH). • Display critical thinking and problem solving skills related to residential program needs.

• Display good judgment and decision making related to all program routines, therapeutic plans of youth, and program rules and protocols.

Requirements and Education:

• HS Diploma/GED required.

• Experience working with emotionally disturbed population.

• A valid, clean driver's license is required.

• Must pass physical.

• Must be able to complete TCI training.

At least six months of related experience is required with demonstrated competency in Level 1 essential job duties. One year of related experience is desired.

As a Director of ACT, you will have the opportunity to significantly improve the lives of adults who have a severe and persistent mental illness that seriously impairs their functioning in the community. You will oversee a multi-disciplinary team that provides services and support to adults living in the community Your duties include the management of the program, staff matters, client referrals, program manual, and program systems. You will promote the agency's commitment to excellence in the provision of mental health services that are strength-based, recovery-oriented, shared decision-making process, along with culturally competent and sensitive. ACT is a program that provides services through less traditional service delivery methods. This is very much a "meet the client in their community" approach. You will report to the Chief Officer of Crisis Response Service and work onsite.

Responsibilities:

• Provide overall supervision to the team and is responsible for allocation of staff time
• Ensure all team members are kept informed of issues involving participants and their families
• Monitor caseload size and coordinates program intakes and discharges
• Facilitate, organize, and document daily team meetings
• Provide training and supervision for the Staff
• Make rotating on-call schedule for after hours / weekend coverage by team members
• Provide after hour and weekend telephonic backup for on-call staff
• Educate the community about Assertive Community Treatment (ACT); philosophy, program outcomes, theoretical background
• Develop and maintain collaborative relationships with other agencies, schools, colleges, institutions, hospitals, primary care providers, housing authorities, and law enforcement agencies to help meet the needs of this population so they can achieve and maintain their independence
• Ensure staff are adhering to the fidelity of the program
• Participate in all required trainings as directed by funding source and ensure that staff are also in compliance with all funding source training
• Be a primary clinician for a subset of program participants. In this role of Primary Clinician, the Team Leader will:
  • Be a point person for the participant and their family
  • Conduct needs assessments
  • Engage in safety planning and wellness management planning with participants
  • Connect participants and families with the services and supports they need
  • Provide psychoeducation regarding mental health diagnosis and treatment
  • Develop and modify treatment using a shared decision-making process with clients

Requirements and Education:

• A Master’s degree and license in a helping profession- Social Work, Mental Health Counseling, Marriage and Family Therapy, Creative Arts Therapy, etc.OR licensure/registration as a RN (RNs are encouraged to be master’s level or nurse practitioner), or physician.
• At least two years of progressively responsible related experience.

Responsibilities

• Maintain the facilities during all seasons in accordance with the preventive maintenance program.
• Perform electrical work. For example: Replace light switches, Reset circuit breakers, Replace wall plug receptacles, Replace fluorescent light ballast, Replace electrical solenoid valves, Replace small motors
• Perform basic plumbing work. For example: Clear clogged sinks, toilets, garbage disposals and drain lines, Repair or replace valves, Replace washer, gaskets, vacuum breakers, toilet seals,
• Perform Preventive Maintenance work. For example: Monitor and maintain all laundry and dry cleaning equipment, Monitor and maintain all kitchen equipment, Change air-conditioning filters.

Requirements:

• 1 year previous experience.
