As a Peer Advocate, you will be an ally, mentor, and advocate for clients and families as they contend with the difficulties and uncertainties of mental illness. You will use lived experience to contribute to treatment planning for each participant, and facilitating the team’s development and maintenance of a culture of respect. The Youth Assertive Community Treatment team (ACT) Program is an innovative treatment program. The program serves children with mental illness or serious emotional disturbance and their families. You will report to the Director of Youth Act and work onsite with community-based travel required.
This new employee will be hired under the Youth Peer Advocate title or Family Peer Advocate title. The requirements are as follows:
Youth Peer Advocate:
- As this is a Youth Peer position, grant funding requires that employees hired in this capacity are within 18-30 years of age and have lived experiencewith mental illness or serious emotional disturbance.
- The FPA must possess a credential recognized by the Office of Mental Health and receive specialized training and supervision.
Family Peer Advocate:
- Family Peer Advocates (FPA) are parents or caregivers who are raising or have raised a child with serious mental health concerns and are personally familiar with the associated challenges and available community resources for children and families.
- The YPA must possess (or obtain within the first twelve months of employment,) a credential recognized by the Office of Mental Health and receive specialized training and supervision.
Peer Advocate Responsibilities:
• Engage individuals and establish trusting, collaborative relationships directed toward the goals clients identify on their treatment plans
• Contribute to client treatment plans, advocating for clients' needs
• Participate in daily treatment team meetings and work directly with other team members to provide support and assistance to program participants.
• Provide individual face-to-face support to youth and families receiving services
• Spend 70% or more of scheduled work hours in community settings
• Meet with family, youth, and collateral contacts to provide psychoeducation about serious mental illness
• Be a role model, educate recipients about self-help techniques and self-help group processes, teach effective coping strategies based on personal experience, teach symptom management skills, assist in clarifying rehabilitation and recovery goals, and assist in the development of community support systems and networks.
Requirements and Education:
- High School Diploma or GED, Associates Degree preferred
- A valid New York State driver’s license and must have access to a reliable, insured vehicle and be willing to transport clients in the community if needed.
- Complete and obtain the NYCPS-Provisional certification for review by the New York Peer Specialist Certification Board (NYPSCB) within the agreed-upon time of start date.

POSITION OVERVIEW
As a Family Support Worker, you will work as part of the Short-term Services Program which was developed as an intensive case-management service to rapidly resolve the concrete and referral needs of families entering the Child Welfare System. By successfully resolving these issues within 90 days of admission, families are at a reduced risk for being referred to Long-term CPS. Services are provided in the office, home, school, and community. You will report to the Coordinator of Short-term Services and will work onsite with community-based travel required.
Responsibilities:
- Manage a full caseload.
- Complete all required trainings, participate in supervision with supervisor and attend required meetings and in-services as directed by supervisor.
- Establish positive and professional relationships with families.
- Model and demonstrate problem-solving and decision-making skills.
- Work with SCDSS case managers to identify service needs of at-risk families.
- Have a thorough knowledge of community services in Schenectady County.
- Help families obtain services they need by making referrals, providing transportation, accompanying them to appointments, assisting with applications (medical, schools, DSS, etc.).
- Maintain good working relationships with DSS/Support Unit caseworkers.
- Complete progress notes or other case record notes promptly.
- Maintain cultural sensitivity, competence and client confidentiality.
- Other duties as assigned.
Requirements and Education:
- Requires an Associate’s degree and one to two years of related experience, or equivalent education and experience.
- Must have a valid NYS driver’s license.

Director Nursing - Sign-On Bonus & Relocation Assistance Available
As a Nursing Director, you will lead medical services delivery within the division of Residential and Foster Care Services for Northern Rivers. This also includes oversight of nursing services provided in the agency schools and day treatment. You will oversee policy and protocol development and implementation for health service delivery, and will work with the Medical Director to ensure medical services are provided according to all applicable standards and regulations. You will report to the Chief Officer, Residential & Foster Care Services and you will work on the Albany campus with expectation of some visits to the Schenectady campus.
Responsibilities:
- Maintain appropriate staffing levels for medical services including hiring, staff development and retention of staff.
- Oversee policy and protocol development and implementation for health service delivery for the division
- Review existing practices to find efficiencies and establish common protocols between programs when possible.
- Develop relationships with health providers, schools of nursing, and possible services that the agency will contract with for healthcare
- Maintain a strong working relationship with agency leadership including chiefs, directors, and central services.
- Oversee management of billing and managed care relationships relative to these health services.
- Oversee compliance of medical records with all applicable standards and implement recommendations of other quality improvement and regulatory bodies (i.e., COA, IOC, OCFS, SED, OMH, etc.)
- Participate as a member of multi-disciplinary teams with residential services, foster care and the education department in support of coordination of care of youth.
- Position takes part in rotating telephonic on call to support Nursing.
Requirements and Education:
- Bachelor's degree (BA/BS) from a four-year college or university
- 5 to 7 years of management or supervisory experience
- RN License in New York required.
- Must pass physical.
- Must be able to complete TCI training.

Description
As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work w/children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review Plan of Care; identifying progress, continued needs, and confirming ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended trainings
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- You have a valid driver's license and automobile that is insured following New York State Requirements
- You will work with individuals of different backgrounds, age, ethnicities, life positions, and socio-economic statuses.
