Credible Messenger Counselor

As a Credible Messenger Counselor, you will develop transformative mentoring relationships with youth in the justice system. Credible Messenger Mentoring for youth in the juvenile justice system is a transformative process through which individuals from similar backgrounds, especially men and women who have personal experience in the justice system, engage youth in structured and intentional relationships that help them change their attitudes, belief, and actions. You will also provide therapeutic and case management services for the youth and the family. You will report to the Assistant Director of Juvenile Justice Programming in this hybrid position.

Responsibilities:

• Maintain a caseload of nine youth and and meet with each youth weekly in their home or a mutually agreed upon community setting, as well as providing outreach phone calls and support to caregivers.
• Advocate and support youth as they interact with justice systems, education, employment, health, and social service systems.
• Participate in Credible Messenger training and consultation opportunities to develop restorative justice and mentoring skills, as well as learning about promoting a lifestyle of sustained transformation.
• Advocate and guide youth as they interact with justice systems, education, employment, health, and social service systems.
• Establish and maintain working relationships with colleagues, Probation Department, service providers, and community resources.
• Maintain case record and complete all paperwork: work-based Learning documentation, status sheets, progress notes, service plans in accordance with program contract, agency policy, COA and state mandates.
• Coordinate and implement work-based Learning interventions with each youth, including the issuing of stipends to the youth and obtaining all necessary signatures to verify accuracy of implementation.

Requirements and Education:

• A Bachelor's degree in Social Work or related field with at least three to five years of experience.
• At least one to two years of demonstrated successful “lived experience” within the Justice or Child Welfare Systems.
• A valid Drivers License, and access to a reliable vehicle to conduct home visits and outreach to youth, as well as transporting youth.

As a Science Teacher, you will develop and implement Science classroom instructional plans for students. You will provide subject area instruction (Science) that follow the NYS K-12 Science Curricula and prepare students for exit examinations and statewide assessments. You will report to the Assistant principal and you will work onsite.

Responsibilities:

• Develop and implement science instructional materials to provide individual and small group instruction to students of various ability levels, and at the high school level develop, teach, and assess laboratory experiences for Earth Science, Living Environment, and/or Environmental Science.
• Develop and implement classroom instructional plans with the Special Education Teachers that integrates the student's IEP goals and objectives, learning styles, and accommodations and modifications that are needed.
• Work with the Special Education Teachers on individual and classroom behavior plans for managing student behavior to maintain discipline in the classroom.
• Use the classroom PBIS universal plan and Tier 2 and 3 strategies for selected students.
• Coordinate the safety and well-being of students through the use of established classroom and schoolwide processes and procedures; supervise classroom staff in implementing these with their work with students throughout the building.

As a Registered Nurse, you will provide nursing care and services to a youth and adolescent population with emotional, behavioral and physical needs using the nursing process (assessing, diagnosing,planning, implementing, and evaluating). You will report to the Nursing Manager and you will work onsite.

Responsibilities:

• Complete the nursing assessment, evaluation and treatment plan of care for the physical and behavioral/emotional health of the residents
• Case management of outside provider visits (medical and dental) including annual assessments
• Review lab and test results, reporting critical values to providers
• Medication management including ordering medications and obtaining prior authorizations
• Dispense medications; report/discuss observed side effects or other concerns with consulting psychiatrist
• Provide client and family education regarding medications and treatments\
• Collaborate with our staff regarding concerns and behaviors

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

• Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
• Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
  • Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
  • Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
  • Work with children, youth, families, and service providers to create a comprehensive Plan of Care
  • Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
  • Convene and conduct Interdisciplinary team meetings to review the Plan of Care; identify progress, and continued needs, and confirm ongoing eligibility
  • Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
  • Use NY State and Health Home information systems to input required Health Home documentation

As a Foster Care Home Finder, you will recruit, train, certify, and maintain foster and pre-adoptive foster homes to children from birth through age 21 who have been removed from their homes and placed in foster care. Each foster parent receives training and ongoing professional support to promote a positive, successful experience. As a Clinician for Mobile Crisis, you will be providing crisis risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), crisis stabilization and interventions, de-escalation techniques and follow-up services to the identified client experiencing emotional, behavioral, and psychiatric crises. These interventions, provided by telephone and on-site mobile visits, will be implemented using a strength-based, trauma-informed, systems-oriented perspective and will include the development of crisis prevention and safety plans; follow-up to evaluate the effectiveness of plans; coordination of services with existing treatment providers; and linkage to additional community-based therapeutic resources and supports. You will directly report to the Clinical Site Supervisors with Director and Executive Program Director guidance and support. You will work on-site with travel to the community and individuals' homes.

Responsibilities:

• Provide rapid response to adults, children, adolescents, and families, who are in crisis, experiencing an emotional or behavioral disturbance by phone and on-site within the community, and as assigned, participate in acute care emergency services rotation system designed to facilitate and strengthen community-based crisis intervention services. This includes rotation of schedule to provide coverage during holidays and weekends.
• Conduct risk assessments with a diagnosis (with appropriate supervisory approval, per scope of practice), mental status examinations, and substance abuse screening.
• Analyze information to identify action plan, which includes crisis stabilization plan, safety, and crisis prevention plans, and plan for follow-up. Consult with team and supervisors.
• Coordinate with community resources for collateral input and follow-up care, including referrals for mental health treatment and substance abuse assessment and treatment.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

• Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
• Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.

As a Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. • Function as a systems navigator to families who have adopted or who have children in guardianship or kinship care through the provision of case management.

Per Diem Clinician, Permanency Resource Center (PRC)

As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicated to individuals, families and groups in a Permanency Resource Center. This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases. Therapy is to be provided in the community or in office, and is paid for by the program, not through insurance reimbursement. You will report to the Licensed Clinician and you will work in a hybrid model with community-based travel required.

Responsibilities:

• Provision of outpatient therapy remotely or in person with youth or caregivers within the Permanency Resource Center system.
• Document the contact within the Evolv system.
• Attend and participate in individual supervision on at least a monthly basis and more frequently if the acuity of the assigned cases requires this.
• Provide therapeutic services to individuals assigned to case load.
• Provide case management services as clinically indicated.
• Provide referral, collateral and linkage services.
• Record and maintain case records according to program standards.

Per Diem (As Needed) Shift

As a Respite Worker for the Permanency Resource Center, you will provide respite services to youth in adoptive, guardianship and kinship families. Respite may also be available to families as part of the service plan and can occur at parent support groups or other PRC events and will be designed to be consistent with the children's needs and interests. Outings or activities with individual youth will be coordinated with the supervisor and parent. Respite provided as part of a group activity at a support group will be designed to allow parents the opportunity to have time separate from their children while they attend a support group. You will report to the Case Manager Supervisor or designated supervisor at the event. You will work onsite with youth in the community.

Responsibilities:

• Provide supervision for youth while parents attend support group.
