Clinicial Site Supervisor - Mobile Crisis

Licensed Social Workers are eligible for a $7,500 signing bonus!

As a mobile Clinical Site Supervisor, you will oversee daily management and oversight of all staff to ensure management of the crisis line and onsite response to all 6 counties is coordinated. You will report to Director of Crisis Services and you will work onsite.

Responsibilities:

• Provide daily supervision and oversight to staff including:
• Manage the crisis line
• Monitor triages to ensure appropriate call management response
• Assist with coordinating team response based on staffing per call
• Maintain knowledge of the schedule for team response
• Monitor Teams Chat and provide supervision/coordination
• Interview, hire, and coordinate the onboarding of new staff with the Director
• Oversee the training of new staff according to each discipline
• Provide daily Clinical Supervision for staff by observing and coaching management of crisis calls. Regular individual supervision
• Collaborate with the Director around concerns or program needs
• Work with program coordinators and case managers on file maintenance, billing, referrals, and response to clients' needs
• Participate in debriefs for critical incidents and review information to determine opportunities for staff development
• Collaborate with the Director on creating team-building opportunities and develop staff to provide cohesive direct care in a team atmosphere
• Act as a liaison to community partners to support program service
• Maintain knowledge of Human Resources policies and procedures
• Participate in advanced training to the position
• Complete Performance Evaluations yearly of supervisees
• Monitor all Documentation of staff as oversite to ensure quality documentation
• Enhance collaborative teamwork between all staff
• Rotate on-call for Mobile Crisis Services; some office evening hours

Requirements and Education:

• Master's degree (MA/MS) required
• 3 - 4 year's experience
• LCSW or LMHC required, LMSW eligible for LCSW will be considered
• Valid NYS Driver's License

Behavioral Health Clinician

MSWs with their Limited Permit are eligible for a $2,500 signing bonus!

As a Clinician for the Behavioral Health Center, you will work as part of an interdisciplinary team with the underserved populations in our community by providing clinical counseling to individuals, families and couples in an outpatient mental health clinic licensed by the New York State Office of Mental Health (OMH).At the Albany County Probation site, you will focus on screening children and adolescents for mental health needs as well as case management and collaboration with the probation team to ensure connection to appropriate services in the community. You will report to the Director of Behavioral Health and will work onsite, with your hours divided between two locations, the Behavioral Health Center at New Karner in Albany and the Albany County Probation Department, Juvenile Services Office.

Responsibilities:

• Manage a caseload of clients aged birth to 65, providing comprehensive assessment, diagnosis, and individual and family therapy services to those assigned to caseload.
• Create initial treatment plans, subsequent treatment plan reviews, progress notes for each scheduled appointment, and other documentation/assessments as indicated.
• Collaborate and coordinate with current community resources and assist with referrals for such services.
• Attend Behavioral Health Center staff meeting twice a month.
• Attend and participate in Supervision; weekly, bi-weekly or monthly based on licensure level or individual needs.
• Hours require one evening a week until 8 pm.

Requirements and Education:

• Requires a Master’s degree, MSW or equivalent.
• Must be licensed prior to starting, or be willing to apply for a permit upon hire. LMSW, LCSW, or LCSW-R preferred.
• Required to pass licensing exam within six months of employment if working with a limited permit.
• Valid, clean driver’s license is required.

Clinical Case Manager, Intensive Family Empowerment Program

As a Clinical Case Manager in the Intensive Family Empowerment Program, you will provide six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and will work in a combination of on-site and remote settings with travel to client's homes required.

Responsibilities:

• Provide home and community services to the identified youth and their caregiver as prescribed in the treatment plan for those enrolled in our Family Preservation Services.
• Educate parents and their child in a variety of social-emotional and parenting skills such as communication, discipline, anger management, emotional regulation, attunement, caregiver affect management, decision making, problem-solving, social skill building, budgeting, family and home organization, behavioral management, etc.
• Maintain a caseload of three children.
• Complete and maintain appropriate case records through the completion of intake, assessments, progress notes, reports, etc. following agency, county, and state policy and funding requirements.
• Participate in the comprehensive assessment and creation of the treatment plan for assigned cases to determine and address service needs and ensure the provision of regular treatment plan meetings and service plan reviews.
• Provide liaison contact with collateral resources such as referring agencies, schools, family courts, and other agency personnel serving on behalf of a child and family as it relates to their treatment.
• You will work Monday through Friday with a flexible schedule that includes evenings and a rotating on-call schedule.

Requirements and Education:

• Bachelor's degree in social work, Sociology, Criminal Justice, Psychology, Human Services, or a related field;
• Experience working with at-risk youth and families;
• Home and community-based experience is a plus.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.

Responsibilities:

• Level I - Provides care management services to children with low and medium acuities. The Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in person with the member.
• Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
  • Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
  • Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
  • Work with children, youth, families, and service providers to create a comprehensive Plan of Care
  • Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
  • Convene and conduct Interdisciplinary team meetings to review the Plan of Care; identify progress, and continued needs, and confirm ongoing eligibility
  • Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
  • Use NY State and Health Home information systems to input required Health Home documentation
  • Comply with Medicaid and program billing regulations
  • Complete all required and recommended training

Requirements and Education:

• Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
• Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
• You have a valid driver's license and automobile that is insured following New York State Requirements
• You will work with individuals of different backgrounds, age, ethnicities, life positions, and socio-economic statuses.
