POSITION OVERVIEW
Clinical Case Manager
Johnstown, NY
The Intensive Aftercare Prevention Program (IAPP) is looking for a Clinical Case Manager to work in a home and community-based program where employees work in a partnership. This position is working in Johnstown, NY and covering FultonCounty.
The Intensive Aftercare Prevention Program (IAPP) provides six to nine months of intensive services to families whose children are either at risk of being removed from their homes or are already in placement. The goals of the program are to prevent out-of-home placement for troubled children, provide a safe environment to return children already in placement, and prevent future out-of-home placement.
What a Clinical Case Manager can expect:
- The Clinical Case Manager will develop treatment plans with families to assist with identifying and meeting their goals.
- Develop a psychosocial assessment. Connect with the youth’s family to monitor progress and support the family’s effort to keep the youth safe.
- Ensure the coordination of services with service providers and collaterals involved with the youth and his/her family.
- Complete paperwork properly and within agency, county, and state guidelines. Ability and willingness to do the job as changes arise.
Work Schedule: Monday-Friday; 8:30AM - 5PM; flexible schedule with evenings. Rotating on-call schedule.
Your Experience and Education:
- Bachelor’s degree in social work, sociology, criminal justice, psychology, human services or related field preferred;
- Experience working with at-risk youth and families;
- Home and community based experience a plus.

Clinician, Intensive Aftercare Prevention Program

As a Clinician in the Intensive Aftercare Prevention program, you will provide home-based clinical services to children and their families to help preserve or reunite them following Northeast Parent & Child Society's stated purpose and mission of protecting children, preserving families, and strengthen communities. The Intensive Aftercare Prevention Program (IAPP) is a home and community-based program assisting families who have children in foster care or children who are at risk of being placed in foster care. You will report to the Program Coordinator and you will work remotely with travel to client homes required.
Responsibilities:
- Provide home and community-based assessment, planning and treatment with individuals, groups and families with those enrolled in our Family Preservation services (the level of intensity / number of direct services hours per family is dependent upon the services in which one is employed)
- Complete necessary all records and client information following agency, county, and state policy and funding requirements
- Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
- Responsible for the clinical decisions when assigned and paired to work as a team with a Bachelors Clinical Case Manager.
- Ensure provisions of regular treatment plan meetings and services plan reviews.
Requirements and Education:
- Requires a Masters degree in Social Work, Mental Health Counseling or related field
- Minimum of one year of post-graduate experience.
- License Preferred: LMSW, LCSW, LMHC, or LMFT
- Must have a valid drivers license.
