Description

As a Clinician, you will coordinate client assessments and work with the child, family, legal guardian, law guardian, psychiatrist, nurse, psychologist, educators, child care staff, and other providers to prepare, implement and monitor a comprehensive, permanency-focused case plan for children in the Residential Program. You will report to the Director of the Residential Treatment Facility and will work on-site.

Responsibilities:

• Provide assessment, treatment planning and general counseling

• Required to provide 20-25 hours of individual, group, or family therapy per week.

• Conduct specialized risk assessments.

• Consult with other staff involved in the care of the child and families.

• Attend case related meetings.

• Participate in supervision.

• Have regular communication with birth families around progress/problems of children in care, how to support them and work towards permanency.

• Provide regular case documentation for all children and families on workload including assessments, progress notes, treatment plans.

Requirements and Education:

• Requires a Master’s degree; related experience preferred.

• RTF Clinician requires two years of related experience.

• Professional licensure:(LMSW with 3 years of experience, LMHC-D, LMFT,-D LCAT, LCSW) required

• Must pass physical.

• Must be able to complete TCI training.

As a Family Support Worker, you will use your unique experience raising or having raised a child with a mental health concern to provide parenting education, support and activities aimed at improving the health and well-being of school-aged children and their families.You will report to the Senior Director of Behavioral Health. You will work onsite, splitting time between Roots Academy at West Hill and Sheridan Preparatory Academy, with some travel required to community-based locations.

Responsibilities:

• Foster positive and collaborative relationships with parents and guardians to promote students’ emotional, physical and behavioral development.
• Provide empathetic, patient, and non-judgmental support to children and families.
• Offer guidance, resources and empowerment to families, helping them understand and address the health needs of both themselves and their children.
• Maintain familiarity with community resources and services available to families.
• Collaborate with community organizations to connect families with additional resources and services in the school community.
• Facilitate family support groups, workshops and parent training sessions focused on mental health awareness, positive parenting strategies and building support systems within their communities.
• Participate in school support and multidisciplinary team meetings to discuss student progress, concerns and intervention strategies.
• Organize and lead student lunch groups and summer activities for students enrolled in school-based behavioral health services.
• Maintain accurate and up-to-date records on student progress, family interactions and support activities.
• Contribute to the school-based behavioral health team through participation in staff meetings and training opportunities.

Requirements and Education:

• High School Diploma or equivalent required.
• 1 or more years of experience working with children in an educational or mental health setting required.
• Personal experience as a parent of a child with mental health needs required.
• Knowledge of child development, mental health disorders, and support strategies for emotional and behavioral challenges.
• Valid NYS driver's license and reliable transportation required.
• Flexibility in work hours required.

You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.

• Work with family members to identify goals and objectives of treatment
• Provide services based on the needs as identified in the Treatment Plan

• Participate in relevant treatment meetings and attend all required trainings
• Complete all documentation

POSITION OVERVIEW

POSITION SUMMARY

Provide targeted skill building activities with youth in community residence program. Skill training to include areas of independent living and social skill activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• At minimum, 50% of work week is spent face to face with clients to provide services designated in the Treatment Plan.
• Provide services based on the youths needs and preferences as identified in the Treatment Plan.
• Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
• Complete all required documentation and submit to the Supervisor within required timeframes.
• Treatment plan goals may include Independent Living or social skills training examples Career Planning, Self-Care, Daily Living, Housing, Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures and using their coping skills when feeling dysregulated.
• Staff need to maintain consistency, provide regular routines and be aware of their own emotions when dealing with the youth in care.
• Engage and form positive relationships with all youth served.
• Maintain respectful and appropriate communication with internal and external customers, including county workers, medical staff, parents etc.
• Develop and maintain relationships with community organizations and consumers to expand opportunities for community connections.
• Facilitate service provision and attend relevant meetings as required.
• Provide information to collateral contacts for the purpose of Treatment planning and assist in tracking progress as required.
• Attend all required trainings and meet regularly for supervision.
• Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with case manager to find resources in the community for educational and vocational skills and programming for discharge.

EDUCATION and/or EXPERIENCE

• High School Diploma/GED required
• At least 2 years of experience

CERTIFICATION / LICENSURE

• Valid drivers license required
