Certified Home Health Aid
Summary: The certified Home Health Aide performs services for the clients as necessary to maintain the personal comfort of the client enabling them to remain safely in their homes.
Essential Job Function/Responsibilities:
1. Patient care a. Understand and adhere to the information and precautions contained in individual client's Home Plan of Care and as direct by the RN assessment nurse. b. Assist the client with personal care activities including: bathing, skin care, hair care, nail care, dressing and undressing, feeding , oral hygiene, shaving, grooming and bedtime care. c. Assist the client to the bathroom or in use of urinal or bedpan. Keep incontinent clients clean and dry. d. Assist the client with client self-administration of medications. e. Assist the client with personal communication skills, as need. f. Prepare appropriate documentation of the client care or services furnished. g. Report all client and employee incident/accidents to the supervisor immediately. h. Assist the client with exercise, ambulation and transfer activities. Be aware of proper body mechanics. i. Maintain proper hand washing techniques. j. Ensure maintenance of clean, safe, comfortable and healthy environment for the client. k. Observe safety precaution including: wiping up wet floors, spills and other falling hazards immediately. l. Performs other related duties as directed.
2. Communication a. Engage in respectful social interaction with the client, including friendly conversation and empathetic support, as well as, respecting client's privacy and property. b. Exhibit positive attitude and behavior and demonstrate respect for employees and clients. c. Maintain absolute confidentiality of all information pertaining to clients. d. Respond and attend to client's request promptly. e. Communicate effectively with all member of the team.
f. Maintains continuity of client’s home care by documenting and communicating actions, irregularities and continuing needs in the client’s journal for the next caregiver. g. Wears an identification badge while caring for the client at all times while in the client's home as mandated the the Division of Consumer Affairs.
3. Additional Duties a. Understand the importance of seeking assistance, as needed , from your supervisor and /or others;demonstrate capability and dependability in following instructions. b. Understand that regular, consistent attendance is necessary to serve clients. c. Accept and fulfill assignments with Grandma’s Home Helpers; exercise judgement in accepting assignments
