Wellness Navigator

AGENCY: INTERIM, Inc. is a private non-profit agency which provides residential treatment, affordable housing, and social rehabilitation programs for adults with psychiatric disabilities.

POSITION: Wellness Navigator

HOURS: Full time or part-time, from 20 hrs./wk. up to 40 hrs. per week; depending upon duties and responsibilities assigned; non-exempt, eligible for overtime.

SALARY: $15.86 - $20.65 per hour (7 steps); If full-time, employee medical/dental/vision coverage paid by agency; if part-time, a portion of medical premium paid by agency; 403(b) savings plans; paid vacation and sick leave.

POSITION OVERVIEW: The Wellness Navigators draw upon their personal, lived experience in mental health in their work as peer providers to assist others by connecting them with resources within the mental health community services network. A Wellness Navigator will serve as a role model to mental health clients; exhibit competency in personal recovery and use of adaptive coping skills; serve as an advocate; provide information and support for outpatient settings. The Wellness Navigator must be able to work with a diverse group of adults; engaging individuals wanting assistance and providing information about community resources. The Wellness Navigator will help create a welcoming environment in the program.


Education: Required: GED or High School Diploma; BA preferred.

Experience: Required: Applicants must have personal, lived experience of recovery from a mental health challenge.

Skills/Knowledge: Required:

 General knowledge of community resources.

 General knowledge and insight into the needs and potential of the population served.

 Knowledge of the recovery process and the ability to promote mental health recovery & evidence-based practices.

 Knowledge and skill to teach and engage in basic problem-solving strategies to support individual clients in self-directed recovery.

 Knowledge of the signs and symptoms of mental health challenges (i.e. auditory and visual hallucinations, aggressive talk and behavior, thoughts of self-harm or harm towards others, isolation).

 Ability to assist the client to address symptoms using strategies such as positive self-talk, and other techniques.

 Flexibility

 Ability to work with a multi-disciplinary team of medical professionals

 Good interpersonal skills

 Good English communication skills, both verbal and written.

 Ability to work one-on-one with clients without supervision.

 Organizational skills and ability to keep detailed records.

 Functional knowledge of Microsoft Office programs (Outlook, Excel, Word).

Desirable: Bi-lingual (English/Spanish or English/Tagalog).

Physical/sensory requirements: Ability to see well enough to read data on a computer screen; ability to hear normal conversations and answer telephone; ability to verbally communicate with clients and staff.

Essential Job Functions

1. Use agency vehicle to transport clients to appointments, meetings and recreational activities as assigned.

2. Assist clients in articulating personal goals for recovery and whole health through the use of one-on-one. During these sessions the Navigator will support clients in identifying strengths to add to client’s healthy habits.

3. Assist clients in working with their case worker and/or psychiatrist to determine steps to take in order to achieve the clients’ goals as part of a self-directed recovery journey.

4. Assist clients in setting up and sustaining a schedule of attendance for self-help support groups, as well as means of locating and joining existing groups and activities to support whole health recovery.

5. Provide client-centered services, focusing on the present. Listen, respond empathically and engage in problem solving.

6. Identify, assess and respond to crisis situations.

7. Serve as a recovery agent by providing and advocating for any effective recovery-based services that will aid the client in daily living.

8. Assist in obtaining services that suit the individual’s recovery needs by providing names of staff, community resources and groups that may be useful. Inform clients about community and natural supports and how to use these in the recovery process. Community resources may include but not limited to: Bienestar Health Clinic, OMNI Resource Center, Supported Education and Employment Services, Central Coast Center for Independent Living, Department of Social Security, housing, etc.

9. Contact clients who are interested in follow-up after their visit to provide additional support and assistance with accessing community resources. This may be done over the phone or meeting the client at a public place or at the desired service.

10. Document the following on the client’s treatment/recovery plan:

12. progress made toward goals

13. Utilize Avatar medical records system, other computer software, and databases.

14. Maintain a working knowledge of current trends and developments in the mental health field by reading books, journals, and other relevant materials; share recovery materials with others at continuing education seminars (and other venues to be developed to support recovery-oriented services); and attend in-service training when offered.

15. Participate in staff meetings: Communicate and collaborate with other staff related to clients’ needs and program, agency and other business.

16. Other duties as assigned.

REQUIREMENTS: Over 18 or minimum 2 years’ driving experience; valid California driver's license; auto in safe operating condition; auto liability insurance; good driving record; criminal record clearance; ability to pass physical exam; verification of citizenship or eligibility to work in the United States as outlined under the Immigration Reform and Control Act of 1986.
