Clinician - Crisis Residence Albany

Our brand new Crisis Residence is looking for an experienced clinician to work with the crisis team  

POSITION SUMMARY
This position functions as a part of a multi-disciplinary crisis team, under the direct supervision of the Program Director. The clinician will provide short term crisis assessment, crisis counseling, treatment planning with behavior management and clinical interventions that identify the client's emotional, behavioral, and psychiatric crisis. The clinician will implement all assessments and treatments using a strength based, trauma informed, systems oriented perspective and will include development of crisis prevention and safety planning for when the client is discharged back to the community. The clinician will also provide case management services such as coordination of services with existing treatment providers and/or linkage to treatment and any additionally community based therapeutic resources  

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. .
 Complete the intake process for admission to the crisis residence
o Risk Assessment - CANS
o Substance Abuse Screen/Assessment
o CSSR
o PHQ9
o Social Needs Screening - Health Leads
o Comprehensive Psychosocial Assessment to include diagnosis and needs to develop treatment plan (IAP)
o Obtain release of information forms for all current  

EDUCATION and/or EXPERIENCE
Requires a Master’s degree and three to four years of related experience.
CERTIFICATION / LICENSURE
Requires LMSW level license / LMHC required; LCSW–R licensure preferred.
