Foster Care and Prevention

As an Administrative Assistant with Foster Care and Prevention, you will perform clerical and administrative functions such as drafting correspondence, scheduling appointments, organizing and maintaining paper and electronic files, and providing information to callers. You will report to the Program Coordinator and will work onsite.

Responsibilities:

• Answer telephones and give information to callers, take messages, or transfer calls to appropriate individuals.
• Operate office equipment such as fax machines, copiers, and phone systems, and use computers for spreadsheet, word processing, database management, and other applications.
• Greet visitors and callers, handle their inquiries, and direct them to the appropriate persons according to their needs.
• Maintain program shared electronic calendars and electronic case records in Evolv.
• Process staff requests for assistance with copying, correspondence, filing and mailings.

As a Teacher Assistant, you will assist the classroom teacher in all aspects of teaching, behavior management, and supporting students success in school. You will report to the Preschool Supervising Teacher and you will work onsite.

Responsibilities:

• Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
• Assist the teacher in devising special strategies for reinforcing material or skills based on an understanding of individual students, their needs, interests, and abilities; create and modify materials to match student abilities.

Residential Assistant Supervisor -

Albany residential has an immediate need for a full-time ResidentialAssistant Supervisor. This position is responsible for supervising direct care staff as well as working as part of the direct care team who collectively provide daily therapeutic care and programming according to agency policy and regulation.

As a Greenhouse Coordinator, you will development and implementation of the botanical classroom instructional plans for students. You will report to the Transition Learning Coordinator and you will work onsite.

Responsibilities:

• Provide subject area instruction (Green house and plan based botanical) that provides students with appropriate level lessons and age experiences.
• Develop and implement instructional materials to provide individual and small group instruction to students of various ability levels grades 8 -12.
• Provide leadership in the school for promoting green house and gardening activities and experiences for students. Work with teachers to encourage classroom, small group, and individual activities, projects, and events.
• Coordinate the selection, ordering, and management of materials and equipment for the greenhouse and greenhouse related activities in conjunction with the school principal.
• Collect and organize program data for grant proposals and reports and assists in writing grants.
• Assist classroom staff with crisis prevention and intervention techniques; assist classroom staff with de-escalation techniques with appropriate; teach students appropriate behavior strategies.
• Represent the greenhouse and gardens at relevant conferences, community events andnetworking opportunities. Conduct workshops, tastings, classroom activities and garden clubs.
• Manage and coordinate the safety and well-being of students through the use of established classroom and schoolwide processes and procedures; supervise classroom staff in implementing these with their work with students throughout the building.
• Follow school and agency procedures and policies for reporting incidents. This includes behavior incident reports (IR) and incidents such as child/sexual abuse, substance abuse, harassment, bullying, and violence.
• Establish relationships with parents and family members to create a partnership around student treatment, student behavior, and academic progress.
• Participate in classroom grade level, and behavior/treatment meetings as well as professional training and development sessions (e.g. TCI, CPR, emergency procedures and other agency requirements.
• Follow all Agency policies and procedures, along with state and federal regulations pertaining to our school and students.
• Facilitate partnership between the greenhouse activities and gardens through home and school education programs by coordinating community workshops, recruiting home garden volunteers.
• Work with other teachers, clinicians, administrators, medical staff and other school staff to provide integrated support, assistance, and treatment to students.
• Assist with supervision of school-to-work program activities as needed.

Requirements and Education:

• Requires a bachelor’s degree in Biology, Environmental Science, or other related field with experience with greenhouse operation.
• Prefer experience teaching in a greenhouse environment.

The Behavioral Health program and the Youth Assertive Community Treatment (ACT) program are seeking a Psychiatric Nurse Practitioner for a shared role working between our Schenectady NY location and our Rensselaer NY location. This position is responsible for providing comprehensive Psychiatric services to adults and youth who are enrolled in our outpatient clinic program and a 28-person caseload of youth enrolled in our Youth ACT program. This position is full-time, 40 hours per week. Must be able to seechildren.

Responsibilities:

• Provide comprehensive psychiatric services to youth enrolled in Youth ACT and adults and youth enrolled in our BehavioralHealth Clinic as assigned
• Provide consultation to staff on diagnostic and treatment questions
• Participate in formal and informal training and education to staff
• Prescribe medications, monitor, and review of drug regimen for enrolled individuals
• Participate in quality assurance activities, consult with the Youth ACT Team in developing and reviewing service plans for all Youth ACT enrolled individuals
• Provide clinical expertise, consultation, and technical assistance to the Youth ACT Team/Behavioral Health, cooperate with the County and participate in any quality audit and utilization review
• Provide on-site/community based/home visits medication evaluations and reviews for Youth ACT enrolled participants.

Education and/or Experience:

• Requires Board Certification by the American Board of Psychiatry and Neurology. Must have a minimum of one year experience working with youth and adults diagnosed with mental illness.
• Must be fully licensed Nurse Practitionerin the State of New York.

You Have, or are willing to get:

• National Provider Identification (NPI) - issued by the federal Center for Medicare and Medicaid Services (CMS). All electronic prescriptions in NYS require an NPI. Claims processing functions also require the NPI of nurse practitioners and physicians.
• Federal Drug Enforcement Administration (DEA) number– issued by the United States Department of Justice Department DEA to nurse practitioners and physicians to prescribe controlled substances. DEA registration must be renewed every three years for those issued a prescriber registration.
• NYS Bureau of Narcotic Enforcement (NYS DOH BNE) registration – issued by the NYS DOH BNE to nurse practitioners and physicians to prescribe controlled substances. NYS DOH BNE registration must be renewed every two years for those issued a prescriber registration.
• NYS Medicaid Enrollment - in cases in which a Prescriber is prescribing to a NYS Medicaid enrolled client. Revalidation must occur at least every five years.

As a Residential Youth Counselor, you will provide daily therapeutic care and programming according to agency policy and governing bodies' (DSS, OMH, JCAHO, etc.) requirements for elementary aged children.. You will work on-site on our Albany campus and will report to the Residential Supervisor.

Responsibilities:

Provide for the physical and psychological needs of clients, including physical safety, hygiene, clothing, food, cooking, medical care, and providing a reliable and nurturing environment.

1:1 Residential Counselor

• You will follow an individual youth’s 1:1 expectations plan, ICMP, and any safety plans in place.

• Engage the youth in activities and conversation that are age appropriate and at the appropriate developmental level.

• Document in writing a summary of each shift.

• Learn the history and specific issues associated with the youth who requires 1:1; will learn how to safely manage any crisis.

• Seek and obtain training in any particular area specific to the youth who requires 1:1care.

• Transport 1:1 client for medical, mental health, and other essential needs.

RC1

• You will must be able to work within trauma informed framework of program, including identifying and using coping/safety plans, maintaining consistency, providing regular routine, and controlling own response to youth.

• Engage and form positive relationship with all youth served.

• Complete all mandatory trainings according to programmatic guidelines.

• Display basic competency in TCI techniques.

• Follow outlined program routines, therapeutic plans of youth, and program rules and protocols.

• Work as part of a team including colleague residential counselors, clinicians, on line oncall, supervisors, and other administrative and functional support staff.

• Maintain respectful and appropriate communication with internal and external customers,including county workers, medical staff, parents etc.

• Complete and maintain documentation to meet programmatic and regulatory standards.

• Transport clients for medical, mental health, and other essential needs.

RC2

All of the above plus the following:

• You will facilitate groups/coached visitation, understanding and applying curriculum and appropriate practice models, i.e. DBT, SAY, etc.

• Take on specific leadership responsibilities for dorm/cottage, including coordinating activities, participating in program-wide committee.

• Must have the ability, skill, and knowledge to take a lead role in a team.

• Form positive relationships with more challenging youth and families.

RC3

All of the above plus the following:

• Display critical thinking and problem solving skills related to residential program needs.

• Display good judgment and decision making related to all program routines, therapeutic plans of youth, and program rules and protocols.

• Lead, delegate, and plan work for other team members.

Requirements and Education:

• HS Diploma/GED required.

• Experience working with emotionally disturbed population.

• A valid, clean driver's license is required.

• Must pass physical.

• Must be able to complete TCI training.

RC 1

Experience with emotionally disturbed population is preferred.

RC 2

At least six months of related experience is required with demonstrated competency in Level 1 essential job duties. One year of related experience is desired.

RC 3

At least one year of related experience is required with demonstrated competency in Level 2 essential job responsibilities. Two years or more related experience is desired.
